噻托溴铵联合布地奈德/福莫特罗对慢性阻塞性肺疾病患者血管内皮功能及T淋巴细胞亚群的影响

【摘要】 目的:探讨噻托溴铵联合布地奈德/福莫特罗对慢性阻塞性肺疾病患者血管内皮功能及T淋巴细胞亚群的影响。方法:将117例慢性阻塞性肺疾病患者按照随机数字表法,分为噻托溴铵治疗组(TB组)、布地奈德/福莫特罗治疗组(BF组)以及噻托溴铵联合布地奈德/福莫特罗治疗组(TCBF组),疗程四周,检测三组患者治疗前后临床疗效、内皮功能及T淋巴细胞亚群变化。结果:治疗后,TCBF组CAT评分、m MRC显著低于TB组、BF组,而FVC、FEV1、FEV1/FVC、6MWT显著高于TB组、BF组(P<0.05);TCBF组NO、VEGF高于TB组、BF组,而ET-1、ES显著低于TB组、BF组(P<0.05);TB组NO高于BF组,而ET-1低于BF组(P<0.05);TCBF组CD3~+、CD4~+及CD4~+/CD8~+显著高于TB组、BF组(P<0.05);TB组CD3~+、CD4~+高于BF组(P<0.05)。相关性分析显示,m MRC、CAT评分及FEV1/FVC与NO、VEGF、ES及CD3~+、CD4~+/CD8~+显著相关。结论:噻托溴铵联合布地奈德/福莫特罗治疗稳定期COPD患者临床疗效显著,可能与改善血管内皮功能、调节T淋巴细胞亚群有关。

【Abstract】 Objective: To investigate the effects of tiotropium bromide combined with budesonide/formoterol in patients with chronic obstructive pulmonary disease and vascular endothelial function of T lymphocytes. Methods: 117 cases of lots of patients with chronic obstructive pulmonary disease were randomly divided into tiotropium treatment group( group TB),budesonide/formoterol treatment group( group BF) and tiotropium bromide combined with budesonide/formoterol treatment group( group TCBF),Treatment for 4 weeks. detected before and after treatment the clinical efficacy of the three groups. Detected the changes of endothelial function and T lymphocyte factor. Results: after treatment,CAT score of TCBF group,m MRC was significantly lower than that of TB group,BF group,FVC,FEV1,FEV1/FVC,6 MWT were significantly higher than that of TB group,BF group( P < 0. 05). After treatment,TCBF group NO and VEGF than in the TB group,BF group,and ET-1,ES were significantly lower than group TB,BF group( P < 0. 05); TB NO group than in the BF group,and ET-1 was lower than that of BF group( P < 0. 05). After treatment,TCBF group CD3,CD4 and CD4/CD8 were significantly higher than that of TB group,BF group( P < 0. 05); group TB CD3,CD4 is higher than that of BF group( P < 0. 05). Correlation analysis showed that m MRC and CAT score FEV1/FVC and NO,VEGF,ES and CD3,CD4 and/CD8 are significantly correlated. Conclusion: tiotropium bromide combined with budesonide/formoterol significantly clinical efficacy in treatment of stable COPD pa-tients,which may improve endothelial function,regulate T lymphocyte factor.
