【摘要】 目的用Meta的方法,探讨高强度聚焦超声(HIFU)治疗结直肠癌肝转移的有效性及安全性。方法将高强度聚焦超声治疗结直肠癌肝转移作为检索关键词,检索中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、维普中文科技期刊(VIP)、万方数字化期刊及MEDLINE、OVID、PUBMED等外文数据库,收集有关高强度聚焦超声治疗结直肠癌转移的临床对照试验(CCTs)。对纳入研究的方法学质量进行评论,然后采用软件Rev Man 5.1.4软件进行Meta分析。结果共纳入8篇随机对照试验,样本量885例,高强度聚焦超声治疗组409例,普通手术组476例。Meta分析结果显示,与普通手术组患者相比,高轻度聚焦超声治疗结直肠癌患者无法继续长时间的生存,研究对象的差异不具备统计学意义,95%CI范围为0.80-1.06,HR为0.92,P为0.25,I2=17%。但在亚组分析中,把两组不同治疗方式进行对比,其中出现的差异符合统计学意义,95%CI范围为0.63-1.00,HR为0.80,P为0.05,I2=0。结论 HIFU在治疗结直肠癌肝转移的效果理想,值得临床广泛应用。更多还原

【Abstract】 Objective to explore effect and safety of high intensity focused ultrasound(HIFU) in treatment of hepatic metastasis of rectal cancer with Meta. Methods with searching keywords of high intensity focused ultrasound in treatment of rectal hepatic metastasis, retrieval CNKI, CBM VIP, Wanfang digital journal and foreign language database of MEDLINE, OVID, PUBMED, collect clinical control tests(CCTs) of high intensity focused ultrasound in treatment of rectal hepatic metastasis. Evaluate quality of methodology included in the study, carry on Meta analysis with software Rev Man 5.1.4. Results 8 randomized controlled tests were included with 885 cases, 409 cases were treated with high intensity focused ultrasound, and 476 cases with routine operation. Meta analysis showed compared with routine surgery group, patients treated with high intensity focused ultrasound can not survive for a long time, difference between subjects showed no statistical significance. 95% CI range was 0.80-1.06, HR was 0.92, P =0.25, I2 = 17%. But in subgroup analysis, comparison different between treatment methods of two groups showed statistical significance. 95%CI range was 0.63-1.00, HR was 0.80, P=0.05, I2 = 0. Conclusion HIFU has ideal effect in treatment of hepatic metastasis of rectal cancer, which is worthy of wide clinical application.更多还原
