丹参注射液辅助治疗Ⅱ型呼吸衰竭的临床疗效及其对动脉血气分析指标、氧化应激反应、炎性反应、血管内皮功能的影响

【摘要】 目的分析丹参注射液辅助治疗Ⅱ型呼吸衰竭的临床疗效及其对动脉血气分析指标、氧化应激反应、炎性反应、血管内皮功能的影响。方法选取2015年2月—2018年2月保定市第一中医院收治的Ⅱ型呼吸衰竭患者108例,根据治疗方法分为对照组和观察组,每组54例。对照组患者采用常规治疗,观察组患者在常规治疗基础上给予丹参注射液辅助治疗;两组患者均连续治疗14 d。比较两组患者临床疗效,治疗前后动脉血气分析指标[包括动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、动脉血氧饱和度(SaO2)]、氧化应激反应指标[包括超氧化物歧化酶(SOD)、脂质过氧化物(LPO)、谷胱甘肽过氧化物酶(GSH-Px)]及降钙素原(PCT)、高迁移率族蛋白B1(HMGB1)、血友病因子(vWF)水平,并观察两组患者治疗期间不良反应发生情况。结果 (1)观察组患者临床疗效优于对照组(P<0.05)。(2)治疗前两组患者PaO2、PaCO2、SaO2比较,差异无统计学意义(P>0.05);治疗后观察组患者PaO2、SaO2高于对照组,PaCO2低于对照组(P<0.05)。(3)治疗前两组患者SOD、LPO、GSH-Px水平比较,差异无统计学意义(P>0.05);治疗后观察组患者SOD、GSH-Px水平高于对照组,LPO水平低于对照组(P<0.05)。(4)治疗前两组患者PCT、HMGB1、vWF水平比较,差异无统计学意义(P>0.05);治疗后观察组患者PCT、HMGB1、vWF水平低于对照组(P<0.05)。(5)两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论丹参注射液辅助治疗Ⅱ型呼吸衰竭的临床疗效确切,可有效改善患者动脉血气分析指标及血管内皮功能、减轻氧化应激反应及炎性反应,且未增加不良反应发生风险。

【Abstract】 Objective To analyze the clinical effect of danshen injection in the adjuvant therapy of type Ⅱ respiratory failure and the impact on arterial blood-gas analysis index,oxidative stress response,inflammatory reaction and vascular endothelial function. Methods A total of 108 patients with type Ⅱ respiratory failure were selected in the First Traditional Chinese Medicine Hospital of Baoding from February 2015 to February 2018,and they were divided into control group and observation group according to therapy,each of 54 cases. Patients in control group received conventional treatment,while patients in observation group received danshen injection based on conventional treatment;both groups continuously treated for 14 days. Clinical effect,blood-gas analysis index(including PaO2,PaCO2 and SaO2),indicators of oxidative stress response(including SOD,LPO and GSH-Px),PCT,HMGB1 and vWF before and after treatment were compared between the two groups,and incidence of adverse reactions was observed during treatment. Results(1)Clinical effect in observation group was statistically significantly better than that in control group(P<0.05).(2)No statistically significant differences of PaO2,PaCO2 or SaO2 was found between the two groups before treatment(P>0.05);after treatment,PaO2 and SaO2 in observation group were statistically significantly higher than those in control group,while PaCO2 in observation group was statistically significantly lower than that in control group(P<0.05).(3)No statistically significant differences of SOD,LPO or GSH-Px was found between the two groups before treatment(P>0.05);after treatment,SOD and GSH-Px in observation group were statistically significantly higher than those in control group,while LPO in observation group was statistically significantly lower than that in control group(P<0.05).(4)No statistically significant differences of PCT,HMGB1 or vWF was found between the two groups before treatment(P>0.05),while PCT,HMGB1 and vWF in observation group were statistically significantly lower than those in control group after treatment(P<0.05).(5)No statistically significant differences of incidence of adverse reactions was found between the two groups during treatment(P>0.05). Conclusion Danshen injection has certain clinical effect in the adjuvant therapy of type Ⅱ respiratory failure,can effectively adjust the arterial blood-gas analysis index,improve the vascular endothelial function,relieve the oxidative stress response and inflammatory reaction,without increasing the risk of adverse reactions.
