静动脉二氧化碳分压差在感染性休克患者液体复苏中的临床应用

【摘要】 目的观察静动脉二氧化碳分压差[P(v-a)CO2]联合上腔静脉血氧饱和度指导感染性休克患者早期液体复苏的治疗效果。方法选取我院2014年3月~2016年3月急诊监护室收治的68例感染性休克患者作为研究对象,随机分为观察组和对照组,每组34例,对两组患者进行液体复苏治疗,两组患者均进行中心静脉及动脉置管,抽取其动静脉血气进行分析,监测中心静脉血氧饱和度,并计算P(v-a)CO2,记录早期液体复苏T0 h、6 h、12 h的乳酸水平及乳酸清除率。结果两组复苏后的乳酸水平明显降低,乳酸清除率明显增加,差异有统计学意义(P<0.05);两组比较复苏后乳酸清除率,观察组明显高于对照组,差异有统计学意义(P<0.05)。结论静动脉二氧化碳分压差[P(v-a)CO2]联合上腔静脉血氧饱和度指导感染性休克患者早期液体复苏效果显著,值得临床推广。

【Abstract】 Objective To observe P(v-a)CO2,partial arterial pressure P(v-a)CO2.The effect of early fluid resuscitation in septic shock patients was guided by differential combined with superior vena cava oxygen saturation.Methods March 2014 in our hospital. 34 patients in each group were treated with fluid resuscitation. The patients in both groups were treated with central vein and arterial catheterization. The arterial and venous blood gas was extracted for analysis and the oxygen saturation of central vein was monitored. The lactate level and the clearance rate of lactate in the early fluid resuscitation were recorded.Results After resuscitation,the lactic acid level in both groups was significantly decreased,and the lactate clearance rate was recorded.The rate was obviously increased,and the difference was statistically significant(P<0.05).The rate of lactate clearance after resuscitation was significantly higher in the observation group than in the control group.Conclusion P(v-a)CO2 combined with oxygen saturation of superior vena cava can guide early fluid resuscitation in patients with septic shock. It is worth popularizing in clinic.
