不同方法修复穿髓型楔状缺损牙的抗折性比较研究
Comparative study on different methods of anti-fracture effects in severe wedge-shaped defect teeth
【摘要】 目的:对比分析不同修复方法治疗穿髓型楔状缺损牙的抗折性能。方法:选择2013年7月~10月在北京回龙观社区卫生服务中心及瑞泰口腔医院就诊患者的160颗穿髓型楔状缺损前磨牙,随机分为4组,每组各40颗牙。经完善的根管治疗后,分别按纤维桩联合复合树脂修复法(A组)、金属桩冠修复法(B组)、纤维桩冠修复法(C组)、全冠修复法(D组)修复;随访4年,评价其抗折性。结果:A组的牙折率小于B、D组,差异均有统计学意义(χ~2=6.13,P<0.05);C组牙折率小于B、D组,差异均有统计学意义(χ~2=4.11,P<0.05);A组与C组牙折率差异无统计学意义(χ~2=0.35,P>0.05);B组与D组牙折率差异无统计学意义(χ~2=0,P>0.05)。A、C组的抗折性均优于B、D组。结论:纤维桩联合复合树脂修复法治疗前磨牙穿髓型楔状缺损的抗折性较强,微创且操作简单,值得临床推广。
【Abstract】 Objective: To compare anti-fracture effects of different methods in severe wedge-shaped defect teeth,and to provide a reliable basis for clinical treatment. Methods: One hundred and sixty premolars with wedge-shaped defect were selected. They were randomly divided into 4 groups. After root canal therapy,teeth in group A were restored with glass fiber post combined with composite resin,group B with metal post-core and metal ceramic crown,group C with glass fiber post and metal ceramic crown,group D with metal ceramic crown. The anti-fracture effectswere evaluated after 4 years follow-up. Results: The fracture rate of group A was less than group B and D,there was statistically significant difference between them,respectively( χ~2= 6. 13,P < 0. 05). The fracture rate of group C was less than group B and D,there was statistically significant difference between them,respectively( χ~2= 4. 11,P < 0. 05). There was no statistically significant difference in the fracture rate between group A and C( χ~2= 0. 35,P > 0. 05). There was no statistically significant difference in the fracture rate between group B and D,there was no difference between them( χ~2= 0,P > 0. 05). The anti-fracture resistance of group A and C were better than group B and D,respectively. Conclusion: Glass fiber post combined with composite resin restoration on premolars with wedge-shaped defect not only has better anti-fracture resistance,but also was a simple and minimally invasive method. The method is worthy of clinical promotion.
