稳定期双相障碍患者静息态脑低频振幅与功能连接研究

Amplitude of low frequency fluctuation and functional connectivity on patients with euthymic bipolar disorder: a resting-state fMRI study

【摘要】 目的:探讨稳定期双相障碍患者静息态脑功能特点。方法:选取17例稳定期双相障碍患者(患者组)和18名健康对照者(对照组)进行静息状态全脑功能磁共振扫描检查;比较两组受试者全脑的低频振幅(ALFF)值的大小差异,然后将两组差异有统计学意义的脑区极值点作为种子点做全脑功能连接并进行组间差异比较。结果:与对照组相比,患者组右侧海马、左侧丘脑、左内侧和旁扣带脑回、左侧补充运动区的ALFF值(P均<0. 01,GRF校正)显著增高[MNI坐标分别为:(39,-18,-15);(-6,-18,3);(-6,3,36);(-12,18,60)];患者组右侧海马与左侧内侧和旁扣带脑回(-3,-15,39)、左侧丘脑与左侧中央后回(-63,-3,27)、右侧中央前回(45,-9,39)以及左内侧和旁扣带脑回和右侧颞中回(60,9,-15)功能连接显著增强(P均<0. 005,GRF校正);患者组左侧补充运动区与右侧缘上回(51,-39,39)功能连接显著增强,与枕叶(brodmann area 17)(-18,-81,9)功能连接显著减弱(P均<0. 01,GRF校正)。结论:稳定期双相障碍患者静息态局部脑区功能活动异常,并存在局部脑功能连接异常。

【Abstract】 Objective: To investigate characteristics of resting-state brain function in patients with bipolar disorder(BD). Method: Resting-state fMRI(rs-fMRI) scans were administered to 17 patients with stable BD,and 18 healthy controls(HCs). The amplitude of low-frequency fluctuation(ALFF) values were compared between the groups,then calculated the functional connectivity(FC) of seeds which are peak points in ALFF significantly different regions with other brain regions,and then significant differences of connectivity between groups were analyzed. Results: Compared to HCs,the BD group showed increased ALFF(P < 0. 01,GRF correction) in the right hippocampus,left thalamus,left midcingulate area,left supplementary motor area[MNI coordinates:(39,-18,-15);(-6,-18,3);(-6,3,36);(-12,18,60) ]. The BD group showed increased FC(P < 0. 005,GRF correction) between right hippocampus and left midcingulate area(-3,-15,39),between left thalamus and left postcentral gyrus(-63,-3,27) 、right precentral gyrus(45,-9,39),between left midcingulate area and right middle temporal gyrus(60,9,-15). Besides,FC between left supplementary motor area and right supramarginal gyrus(51,-39,39) increased in BD group and FC between left supplementary motor area and occipital lobe(brodmann area 17)(-18,-81,9) decreased in patients with BD compared with healthy controls(P < 0. 01,GRF correction). Conclusion: Abnormal activities exist in the brain regions in patients with stable bipolar disorder in the resting state,and the abnormal FC are also existence in local brain regions.
