【Abstract】 Objective To explore the effect of management of chronic disease model on the self efficacy of chronic disease management in patients with chronic disease after discharge,in order to provide evidence for improving self-management ability of patients with gout. Methods One hundred and twenty patients with gout were randomly divided into the control group and the experimental group. The experimental group was implemented for 6 months on the basis of followup. The two groups of patients were evaluated by application of chronic disease management self-efficacy scale on 1 day before discharge,3 months after discharge,6 months after discharge.
