【摘要】 目的初步探讨自发荧光支气管镜(AFB)在中央型支气管肺癌患者诊治中的作用及其临床意义。方法回顾性分析采用AFB联合白光支气管镜(WLB)检查的72例疑诊支气管肺癌的患者,比较两组患者的AFB和WLB镜下异常表现及病理结果,分别统计二者在中央型肺癌诊断中的敏感度和特异度。结果 72例患者中,获得明确病理诊断的共有61例,其中包括重度不典型增生者3例,小细胞肺癌者19例,非小细胞肺癌者39例(鳞癌者25例,腺癌者11例,未分型癌者3例); AFB镜下观察异常者57例,经病理证实的阳性患者54例,; WLB镜下观察异常者46例,经病理证实者39例,二者对中央型肺癌的诊断敏感度分别为88. 5%和63. 9%,差异有统计学意义(P <0. 05);特异度分别为78. 6%和61. 1%,差异无统计学意义(P> 0. 05)。结论 AFB联合WLB能明显提高对中央型支气管肺癌的诊断率,尤其是有助于早癌的诊断。更多还原

【Abstract】 Objective To investigate the value of using autofluorescence bronchoscopy( AFB) for the detection of central airway lung cancer. Methods Totally 72 patients suspected of lung cancer were examined by AFB and white light bronchoscopy( WLB). All abnormal findings were recorded and biopsies were taken for pathological examination. Finally,the sensitivity and specificity of the two methods were compared. Results Among the 72 patients,61 were classified as positive by pathology,including 3 severe dysplasia,19 small cell lung cancer and 39 non-small cell lung cancer which included 25 squamous carcinoma,11 adenocarcinoma and 3 without classification. AFB was used to detect 57 abnormal cases and 54 cases were pathologically confirmed as positive. WLB was used to detect 46 cases and 39 cases were confirmed positive. The relative sensitivity of AFB and WLB in the detection of central airway lung cancer was 88. 5 % and 63. 9 %,respectively,with statistical significant difference( P < 0. 05). The specificity was 78. 6% vs 61. 1%,without statistical difference( P > 0. 05). Conclusion The sensitivity of AFB combined with WLB was better for the diagnosis of central airway lung cancer,especially helpful for the early diagnosis of lung cancer.更多还原
