【摘要】 目的探讨CT引导下肺穿刺注射医用胶在肺部小结节(small pulmonary nodule,SPN)胸腔镜手术前定位的可行性和临床价值。方法 21例21个肺内孤立性结节病灶,术前均行CT引导下经皮肺穿刺注射医用胶定位标记,术中先行肺楔形切除术切除病灶并根据病理结果决定进一步手术方案。结果术前CT引导下经皮肺穿刺注射医用胶定位成功率100%(21/21)。穿刺定位后出现刺激性咳嗽6例(28.6%),无症状气胸5例(23.8%),无出血、血胸。术中均能准确定位后行肺楔形切除术,病理证实为肺癌15例,良性病变6例。结论胸腔镜术前CT引导下经皮肺穿刺注射医用胶定位SPN快速、安全,值得临床推广使用。更多还原

【Abstract】 Objective To evaluate the feasibility and clinical value of CT-guided percutaneous injection of medical glue for preoperative localization before video-assisted thoracoscopic surgery in patients with small pulmonary nodules. Methods A total of21 cases of solitary pulmonary nodules were marked by percutaneous medical glue injection under CT guidance preoperatively. They were given wedge-shaped excision of lung to remove lesions intraoperatively,and then the next operation was based on the pathological results. Results The success rate of preoperatively CT-guided percutaneous injection of medical glue was 100%( 21/21). There were 6 cases of puncture irritating cough( 28. 6%) and 5 cases of asymptomatic pneumothorax( 23. 8%). No bleeding or hemothorax happened. The wedge resection of lung lesion was performed after accurately intraoperative positioning. There were 15 cases of lung cancer and 6 cases of benign diseases confirmed pathologically. Conclusion The CT-guided percutaneous medical glue injection for preoperative localization before video-assisted thoracoscopic surgery is time-saving,safe,and effective in the treatment of small pulmonary nodules,being worthy of clinical use.更多还原
