【摘要】 [目的]探讨血清B型尿钠肽(BNP)、心肌肌钙蛋白T(c Tn T)检测在老年髋部骨折围手术期心脏评估的意义。[方法]2010年1月~2015年1月于本院进行治疗的142例髋部骨折患者分为低龄组和高龄组,将两组患者术前24 h及术后1、3 d血清BNP、c Tn T水平进行检测及比较。[结果]两组BNP检测结果显示:低年龄组及高年龄组术后1、3 d均较术前1 d升高,差异有统计学意义(P<0.05),高年龄组在相同时间点明显高于低年龄组,尤其是术后变化更加明显,相同时间点两组间比较差异有统计学意义(P<0.05)。两组c Tn T测量结果显示:低年龄组及高年龄组术后1、3 d均较术前1 d升高,差异有统计学意义(P<0.05),术后高年龄组在相同时间点明显高于低年龄组,术后3 d两组间比较差异有统计学意义(P<0.05),术前1 d、术后1 d高年龄组有升高趋势,但相同时间点两组间比较差异无统计学意义(P>0.05)。患者年龄与血清BNP、c Tn T检测值的Pearson相关分析显示具有一定相关性。[结论]血清BNP、c Tn T是敏感的心肌梗死及心力衰竭标志物,二者在老年髋部骨折围手术期心脏评估中动态变化价值较高,骨折患者随其年龄及手术升高变化,具有一定规律性。更多还原

【Abstract】 [Objective] To explore significance of brain natriuretic peptide(BNP) and cardiactoponin T(c Tn T) for cardiac evaluation in elderly patients with hip fracture. [Methods] Of the 142 elderly patients with hip fracture who were surgical treated in our department from January 2010 to January 2015, 132 patients were included into this study. In term of age, 69 patients were older than 60 years and younger than 70 years, while the remaining 63 patients were equal to or more the 70 years. The serum BNP and c Tn T were detected before operation, at 1 day and 3 days after operation, in addition, compared between the two groups. [Results] The levels of both serum BNP and c Tn T significantly increased after operation in the two groups, specially in elderly patients, with statistical differences(P<0.05). The ≥70-year groups was significantly superior to the <70-year group in serum BNP and c Tn T at each time point, especially BNP after operation(P<0.05). [Conclusion] Serum BNP and c Tn T, sensitive markers for predicting heart failure and infarction, might be used as a tool for perioperative evaluation ofrisk of heart problem in the elderly patients with hip fracture.更多还原
