三维肛门直肠测压在大便失禁治疗中的应用
Application of Three Dimensional Anorectal Manometry in Treatment of Fecal Incontinence
【摘要】 目的:探讨三维高分辨肛门直肠测压在生物反馈联合电刺激治疗大便失禁患者的肛门直肠功能的评估价值。方法:对107例大便失禁的患者行生物反馈联合电刺激治疗,治疗前后给予三维高分辨肛门直肠测压检测,评定患者肛门直肠压力和直肠感觉的变化。结果:治疗3个疗程后肛管最大静息压(64.6±2.3)mmHg(1 mmHg=0.133 kPa),肛管平均静息压(58.5±2.3)mmHg,肛管最大收缩压(122.0±6.3)mmHg和收缩持续时间(18.7±0.3)s明显高于治疗前(56.8±2.6)mmHg、(51.8±2.6)mmHg、(98.4±5.5)mmHg和(17.1±0.5)s,肛门直肠区域的压力形态明显改善。结论:生物反馈联合电刺激是治疗大便失禁的有效的治疗手段。三维高分辨肛门直肠测压能够有效的发现,定位和评估盆底肌肉舒缩的压力变化。
【Abstract】 Objective To evaluate the value of three dimensional high resolutions anorectal manometry(3D-HRAM) in anorectal function in patients with fecal incontinence treated by biofeedback combined with elec-trical stimulation.MethodsOne hundred and seven patients with fecal incontinence were treated with bio-feedback combined with electrical stimulation, and3D-HRAM was performed before and after treatment.ResultsThe maximum anal resting pressure after 3 courses of treatment(64.6 ± 2.3) mmHg(1 mmHg=0.133 kPa), average anal resting pressure(58.5 ± 2.3)mmHg, anal maximal contraction pressure(122 ± 6.3) mmHg and contraction duration(18.7 ±0.3) s, were signifi-cantly higher than those before treatment [(56.8 ±2.6) mmHg,(51.8 ± 2.6) mmHg,(98.4±5.5) mmHg and(17.1±0.5) s,respectively]. The form of pressure of the anorectal region was improved significantly.ConclusionsBio-feedback combined with electrical stimulation is an effective treatment for fecal incontinence, the 3D-HRAM isan effective method to detect, locate and evaluate the pressure changes of pelvic floor muscles.
