感染高危因素患者在普外科Ⅰ类切口手术围手术期应用抗菌药的必要性探讨

【摘要】 目的探讨对于有感染高危因素的患者在普外科Ⅰ类切口手术围手术期预防应用抗菌药物的必要性。方法在滨州市人民医院普外科Ⅰ类切口手术具有高危因素的患者中,选取2010年6月~2012年6月围手术期预防应用抗菌药物的176例病例作为对照组;选取2012年6月~2014年6月围手术期未应用抗菌药物的158例病例作为试验组进行比较,观察两组患者切口感染率在相应高危因素中有无差别。结果两组患者切口感染率在相应感染高危因素中无明显差异(P>0.05);当多种感染高危因素并存时,两组切口感染率有显著差异(P<0.05)。结论Ⅰ类切口手术围手术期患者,如合并多个感染高危因素,可考虑预防性应用抗菌药物。

【Abstract】 Objective After the March 2012, the "clinical use of antibiotics special rectification program of activities " is issued by the Ministry of Health,then to figuer out if it is necessary to provide perioperative antibiotic use for patients with high risk factors undergoing Type I incision surgery in department of general surgery through a retrospective analysis. Through a retrospective analysis, it is to provide a basis for General Surgery class Ⅰ incision perioperative prophylactic use of antibiotics for patients with risk factors for infection. Methods Among the patients with high risk factors for Type I incision surgery in Binzhou People’s Hospital Department of general surgery, 176 cases of antimicrobial prophylaxis during perioperative period from June 2010 to June 2012 were selected as the control group, and 158 cases without perioperative use of antibiotics from June 2012 to June 2014 were selected as test groups, we analyzed if there was difference in incision infection rate of the two groups in different infection risk factors. Results Wound infection in two groups had no significant difference in the corresponding risk factors(P > 0.05); When multiple risk factors for infection existed, the incision infection rate of the two groups were significantly different(P< 0.05). Conclusion Perioperative prophylactic antibiotics use is not necessary in all patients undergoing type I incision surgery in department of general surgery. If the patient had two or more risk factors for infection existed, prophylactic antibiotics use should be considered; Otherwise, we do not advocate prophylactic antibiotics use in patients who have risk factors in type I incision surgery in department of general surgery.
