2型糖尿病绝经后女性患者骨代谢标志物与骨密度的关系研究

【摘要】 目的探讨2型糖尿病绝经后女性患者的骨代谢标志物与骨密度的关系。方法收集159例2型糖尿病绝经后女性患者的临床资料,根据骨密度将其分为骨量正常组(n=48)、骨量减少组(n=58)、骨质疏松组(n=53)。比较三组患者的血钙、血磷、25-OH-D3、PTH、BGP、PICP、尿NTX等指标。结果骨质疏松组患者的BGP、PICP、尿NTX水平高于正常组和骨量减少组,差异有统计学意义(P<0.05)。三组患者的血钙、血磷、25-OH-D3、PTH水平比较,差异无统计学意义(P>0.05)。Pearson相关性分析显示,2型糖尿病绝经后女性患者的骨密度与BGP、PICP、尿NTX水平呈明显负相关。结论 BGP、PICP、尿NTX水平能反映2型糖尿病绝经后女性患者骨密度,检测血BGP、PICP、尿NTX及骨密度有助于预测2型糖尿病绝经后女性患者骨质疏松的风险。

【Abstract】 Objective To investigate the relationship between bone metabolic markers and bone mineral density in postmenopausal women with type 2 diabetes mellitus. Methods Clinical data of 159 cases of postmenopausal women with type2 diabetes mellitus were collected. All the patients were divided into normal bone mass group(n=48), decreased bone mass group(n=58) and osteoprosis group(n=53) according to the bone mineral density. The levels of serum calcium, phosphorus,25-OH-D3, PTH, BGP, PICP and urinary NTX were compared between the three groups. Results The levels of BGP, PICP and urinary NTX in the osteoprosis group were significantly higher than those in the normal group and decreased bone mass group(P<0.05). There were no significant differences in serum calcium, phosphorus, 25-OH-D3 and PTH levels between the three groups(P>0.05). Pearson correlation analysis showed that bone mineral density in postmenopausal women with type 2 diabetes was negatively correlated with BGP, PICP and urinary NTX levels. Conclusion Levels of BGP, PICP and urinary NTX can reflect bone mineral density in postmenopausal women with type 2 diabetes mellitus. Detection of BGP, PICP and urinary NTX and bone mineral density can help predict the risk of osteoporosis in postmenopausal women with type 2 diabetes mellitus.
