【摘要】 目的研究右美托咪定在不停跳冠状动脉旁路移植术（CABG）后低氧血症中的临床应用。方法 43例不停跳CABG后出现低氧血症的患者随机分成对照组（22例）和实验组（21例）,对照组采用常规治疗,实验组在常规治疗的基础上加上右美托咪定治疗。分别记录治疗前,治疗后30 min、6 h的患者氧饱和度（SpO₂）、心率（HR）、血压（BP）、呼吸频率（RR）、二氧化碳分压（PaCO₂）、氧分压（PaO₂）;观察ICU滞留时间及再次气管插管率。采用SPSS19.0软件对数据进行统计学分析。结果治疗前两组一般资料差异无统计学意义（P>0.05）,治疗后30 min、6 h,实验组的血氧饱和度（SaO₂）和PaO₂高于对照组,两组的SaO₂、HR、PaO₂、差异有统计学差异（P<0.05）,实验组再次插管数低于对照组（P<0.05）,ICU滞留时间低于对照组（P<0.05）。结论右美托咪定可改善患者术后低氧血症,减少再次插管的概率,并可缩短ICU滞留时间。更多还原

【Abstract】 Objective To investigate the effect of dexmedetomidine in postoperative hypoxemia after off-pump coronary artery bypass graft（CABG）. Methods Forty-three patients with hypoxemia after off-pump CABG surgery were randomly divided into two groups,a control group（n=22）and an experimental group（n=21）. Patients in both groups accepted routine treatment. The experimental group was given dexmedetomidine additionally. Oxygen saturation（SpO₂）,heart rate（HR）,blood pressure（BP）,respiratory rate（RR）,partial pressure of carbon dioxide（PaCO₂）,and partial pressure of oxygen（PaO₂）were recorded pre-treatment as well as 30 min and 6 h after treatment. Duration of ICU stay and endotracheal re-intubation rate were also observed. The data were analyzed by SPSS 19.0 software. Results There were no significant differences of general data between two groups before operation（P>0.05）. After treatment for30 min and 6 h,the values of SaO₂ and PaO₂ were improved of the experimental group（P<0.05）,while the HR,SBP and RR decreased compared with each index of control group（P<0.05）. The incidence of endotracheal re-intubation and the duration of ICU stay of the experimental group were lower than those of the control group（P<0.05）. Conclusion Dexmedetomidine treatment could improve postoperative hypoxemia after off-pump CABG,reduce the incidence of endotracheal re-intubation,and shorten the duration of ICU stay.更多还原
