血清肿瘤标志物联合检测在卵巢良恶性疾病鉴别诊断中的应用

【摘要】 目的探究血清肿瘤标志物联合检测在卵巢良恶性疾病鉴别诊断中的应用价值。方法选取2015年1月—2016年10月收治的卵巢疾病患者128例,按疾病类型不同分为卵巢良性肿瘤组(56例)、卵巢癌组(43例)、卵巢囊肿组(29例)。观察各组血清肿瘤标志物[血清癌胚抗原(Carcino-embryonic antigen,CEA)、甲胎蛋白(Alpha-Fetoprotein,AFP)、糖类抗原125(Carbohydrate antigen 125,CA125)、糖类抗原199(Carbohydrate antigen 199,CA199)、糖类抗原72-4(Carbohydrate antigen 72-4,CA72-4)]单项与联合检测价值。计量资料组间比较采用t检验,计数资料比较采用χ~2检验,P<0.05为差异有统计学意义。结果卵巢良性肿瘤组和卵巢囊肿组血清AFP、CEA、CA125、CA72-4水平明显低于卵巢癌组(均P<0.05),CAl99水平高于卵巢癌组(P<0.05);与CEA、AFP、CA125、CA199、CA72-4单项检测相比,5项标志物联合检测阳性率明显提高,差异有统计学意义(均P<0.05);卵巢良性肿瘤与卵巢癌中AFP、CEA特异度较高,CA72-4、CAl25敏感度较高;卵巢良性肿瘤与卵巢囊肿中AFP、CEA特异度较高,CAl99敏感度较高。结论 CA72-4和CAl25是诊断卵巢癌的理想肿瘤标志物,在筛查和诊断卵巢疾病时应联合5项标志物共同检测,以提高诊断准确率。

【Abstract】 Objective To explore the value of combined detection of serum tumor markers in differential diagnosis of benign and malignant ovarian diseases.Methods A total of 128 patients with ovarian diseases were selected from January 2015 to October 2016. They were divided into benign ovarian tumor group(56 cases), ovarian cancer group(43 cases) and ovarian cyst group(29 cases) according to the type of disease. The value of single and combined detection of serum tumor markers in each group(CEA,AFP,-CA125,CA199,CA72-4) were detected. Measurement data were compared by t test and enumeration data were compared by χ~2 test.P<0.05 was considered statistically significant.Results The levels of AFP, CEA, CA125 and CA72-4 in ovarian benign tumor group and ovarian cyst group were significantly lower than those in ovarian cancer group(all P<0.05), and the level of CAl99 was higher than that of ovarian cancer group(P<0.05).The positive rate of combined detection of 5 markers was significantly higher than single detection of CEA, AFP, CA125, CA199 and CA72-4(all P<0.05).The specificity of AFP and CEA is higher in ovarian benign tumor and ovarian cancer, and the sensitivity of CA72-4 and CAl25 is higher. The AFP and CEA specificity of ovarian benign tumor and ovarian cyst is higher, and the sensitivity of CAl99 is higher.Conclusion CA72-4 and CAl25 are satisfactory tumor markers in the diagnosis of ovarian cancer. In screening and diagnosis of ovarian diseases, the joint detection of five markers can increase the diagnostic accuracy.
