颈动脉超声评估颈动脉粥样硬化斑块易损性的临床价值

【摘要】 目的探讨颈动脉超声评估颈动脉粥样硬化斑块易损性的临床价值。方法选择2012年3月至2016年3月在深圳市龙华新区人民医院接受颈动脉内膜剥脱术治疗的41例颈动脉粥样硬化易损性斑块、10例颈动脉粥样硬化稳定性斑块患者作为研究对象。所有患者均采用颈动脉超声法对动脉粥样硬化斑块进行易损性评估,分析颈动脉超声评估动脉粥样硬化斑块易损性的灵敏度(sensitivity,SEN)、特异性(specificity,SPE)、阳性预测值(positive predictive value,PPV)、阴性预测值(negative predictive value,NPV)、评估准确度等指标。结果 (1)51例患者中,颈动脉超声分型为易损性斑块43例(84.3%)、稳定性斑块8例(15.7%)。(2)颈动脉超声评估斑块易损性的SEN、SPE、PPV、NPV、评估准确度分别为97.6%、70.0%、93.0%、87.5%、92.2%。(3)颈动脉超声与临床确诊方法评估颈动脉粥样硬化斑块易损性具有良好的一致性(Kappa=0.927,P<0.05)。结论颈动脉超声评估颈动脉粥样硬化斑块易损性的性能良好,且与临床确诊方法一致性良好。

【Abstract】 Objectives To evaluate the clinical value of carotid artery ultrasonography in assessing the vulnerability ofcarotid atherosclerotic plaque. Methods From March 2012 to March 2016 in The People′s Hospital of LonghuaDistrict,41 patients with vulnerable carotid atherosclerotic plaque and 10 patients with stable carotid atheroscleroticplaque who had been treated by carotid endarterectomy were selected as the objects of this study. Carotid arteryultrasonography was used to assesse vulnerability of carotid atherosclerotic plaque in all the patients. Sensitivity(SEN),specificity(SPE),positive predictive value(PPV),negative predictive value(NPV)and evaluation accuracyof carotid artery ultrasonography to assesses atherosclerotic plaque vulnerability were analyzed. Results Among the51 patients,43 cases(84.3%)with vulnerable plaque were diagnosed by carotid artery ultrasonography. SEN,SPE,PPV,NPV and evaluation accuracy of carotid artery ultrasonography in assessing plaque vulnerability were 97.6%,70%,93%,87.5%,92.2%,respectively. Carotid artery ultrasonography and clinical diagnosis method to assess carotidatherosclerotic plaque vulnerability had good consistency(Kappa=0.927,P<0.05). Conclusions Carotid arteryultrasonography is a good method to evaluate the vulnerability of carotid atherosclerotic plaque,and the results areconsistent with the clinical diagnosis.
