【摘要】 目的探讨缺血修饰白蛋白(IMA)和心肌型脂肪酸结合蛋白(H-FABP)联合检测在急性有机磷中毒(AOPP)早期对心肌损伤诊断的意义。方法选择AOPP患者159例作为中毒组根据中毒程度分轻度、中度和重度,其中轻度73例,中度52例,重度34例。健康体检者60例为对照组。中毒组所有患者于入院1h及入院后6h和12h采集不抗凝静脉血两管,各5ml。一管用于血清肌红蛋白(Mb),肌钙蛋白I(c Tn I)和肌酸激酶同工酶(CK-MB)的检测,另一管检测IMA和H-FABP。对照组于体检当日清晨同样抽取不抗凝静脉血两管,各5ml,用于检测IMA和H-FABP。结果入院时中度和重度中毒组IMA和H-FABP水平较对照组有升高(P<0.05;P<0.01);中度和重度中毒组入院12h IMA和H-FABP水平较入院1h上升明显(P<0.01),与入院6h IMA和H-FABP水平比较也有上升(P<0.05)。结论 IMA和H-FABP对AOPP患者心肌损伤的早期诊断有重要的临床价值。更多还原

【Abstract】 Objective To investigate the change of the ischemia modified albumin(IMA) and myocardial fatty acid binding protein(H-FABP) in acute organophosphate poisoning(AOPP) and the significance in early diagnosis of myocardial injury. Methods Choose 159 cases of AOPP patients as poisoning group,including 73 cases of mild and moderate 52 cases,severe 34 cases.Physical examination 60 cases as control group. All of the poison ing pati-ents after admission 1 h and 6 h and 12 h to collect no anticoagulant venous blood two tube. A tube used in serum myoglobin(Mb),troponin I(cTnI) and creatine kinase isoenzyme(CKMb) detection,another tube detec-tion IMA and H-FABP. Results On moderate and severe poisoning group IMA and H-FABP levels higher than control group(P<0.05,P<0.01);Moderate and severe poisoning group after admission 12 h IMA and H-FABP levels are obviously increased compared with admitted to hospital 1 h(P<0.01) and 6 h after admission(P<0.05). Conclusion IMA and H-FABP in the early diagnosis of myocardial injury in AOPP patients has important value.更多还原
