数字评分法(NRS)与口述评分法(VRS)在老年慢性疼痛患者中的比较

【摘要】 目的:比较数字评分法(numerical rating scale,NRS)与口述评分法(verbal rating scale,VRS)在老年患者疼痛程度评价中的有效性。方法:随机选取2015年在中日友好医院疼痛科住院治疗的老年慢性疼痛患者210例,在护士指导下分别采用NRS和VRS对自身总体、静息和活动时的疼痛程度进行评价,评价完成后护士询问患者更喜欢哪种疼痛评价方法。结果:入组患者平均年龄73.5岁,80岁以上者占37.2%,有轻中度认知障碍者占54.8%,疼痛持续时间>6个月者占70.5%;94.3%的患者能够运用NRS完成疼痛自我评价,所有患者均能运用VRS完成疼痛程度评价;患者静息时的平均NRS评分明显低于总体和活动时;采用VRS评分时,45.2%的患者静息时无痛,38.6%的患者活动时有重度疼痛;NRS与VRS在总体、静息和活动时均具有较强相关性;71.9%的患者更喜欢采用VRS进行疼痛评价,明显高于NRS。结论:NRS和VRS均能有效评价老年患者的疼痛程度,VRS更适于老年患者。

【Abstract】 Objective: To compare the effectiveness of numeric rating scale(NRS) and verbal descriptor scale(VRS) in pain intensity assessments in elderly patients with chronic pain. Methods: Two hundred and ten elderly patients with chronic pain hospitalized in the Department of Pain Medicine of China-Japan Friendship Hospital in 2015 were randomly selected and instructed to measure the intensity of the primary pain experienced by both NRS and VRS for 3 times: a measure of overall pain intensity, and two measures of pain intensity at rest and during movement. After measuring the intensity of the primary pain, the patients were asked about their preference for one of the two scales. Results: Among the 210 seniors who participate, the average age was 73.5 years. Those aged 80 and older accounted for 37.2% of the sample and 54.8% had mild to moderate cognitive impairment, 70.5% had been experiencing pain for 6 months or more. It was found that 94.3% of the elderly were able to make judgment with NRS and 100% of them by VRS. The average pain intensity NRS scores during rest was significantly lower than the scores attributed in the overall assessment and during movement. In judging the intensity of primary pain through VRS, 45.2% of seniors reported no pain at rest, while 38.6% of the elderly reported strong pain during movement. There was a significant association between NRS and VRS in the three assessments and 71.9% of the patients preferred to use VRS to evaluate the pain intensity. Conclusion: While both NRS and VRS can be used effectively to evaluate the intensity of pain in elderly patients with chronic pain, VRS seems to be more suitable.
