【摘要】 目的探讨高龄腹股沟疝合并慢性阻塞性肺病(COPD)患者行无张力疝修补术的护理方法。方法收集2010年4月至2014年7月,黄石市第二医院90例采用无张力疝修补术治疗的高龄腹股沟疝合并COPD患者,术前给予充分评估与准备、术中合理干预、术后监测病情变化、给予合适饮食、药物指导、防治并发症等。结果 90例患者行腹股沟疝无张力补片修补术,术后局部疼痛轻,术后患侧髋关节无牵拉感,均能自由屈伸。所有患者均术后1 d下床活动,没有患者发生COPD急性发作并均痊愈出院。手术时间为40~50 min,平均住院为4~7 d,所有患者均未发生肠梗阻、疝复发、皮下窦道、切口感染等并发症。结论加强对高龄腹股沟疝合并COPD患者的围手术期护理,能有效提高手术疗效、改善患者预后。更多还原

【Abstract】 Objective To explore nursing intervention methods in treatment of elderly patients with inguinal hernia complicated with COPD receiving tension-free hernia repair. Method Ninety elderly patients with inguinal hernia complicated with COPD receiving tension-free hernia repair were collected in April 2010 to July 2014 in the Second Hospital of Huangshi. The patients received adequate preoperative evaluation and preparation,reasonable intraoperative intervention,postoperative condition monitoring,proper diet and medication,and prevention of complications. Results Ninety patients receiving tension-free hernia repair did not use pain pumps and potent analgesics,and did not have postoperative ipsilateral hip stretch.All the patients were free to flexion and extension. All patients return to physical activity at 1 day after operation,and were discharged without acute exacerbation of COPD. Operation time was 40-50 min,and average length of stay were among 4-7 days. No patients had intestinal obstruction,hernia recurrence,subcutaneous sinus,wound infection and other complications. Conclusion Strengthening perioperative nursing intervention for elderly patients with inguinal hernia complicated with COPD could effectively increase surgery efficacy,and improve the prognosis which were worthy to be promoted.更多还原
