北京地区2015年11至12月人呼吸道合胞病毒与人偏肺病毒感染及遗传特征研究
The infection status and genetic feature of human respiratory syncytial virus and human metapneumovirus in Beijing
【摘要】 目的了解2015年11~12月北京地区流感样病例中人呼吸道合胞病毒与人偏肺病毒感染及病毒基因遗传特征。方法选取2015年11~12月北京市海淀、通州、顺义区和西城区的874例流感样病例,收集病例的临床资料,采集咽拭子样本,应用实时荧光定量PCR法进行人呼吸道合胞病毒、人偏肺病毒核酸筛查;病毒核酸阳性的标本进行呼吸道合胞病毒和偏肺病毒特异性G基因扩增,扩增产物测序后经序列比对;采用MEGA5.0软件进行基因进化树构建和分析。结果 874份样本中,人呼吸道合胞病毒核酸阳性样本为19份,阳性率为2.17%(19/874);人偏肺病毒核酸阳性14份,阳性率为1.60%(14/874)。人呼吸道合胞病毒阳性检出率在15岁以下患者组为5.43%(17/313),明显高于15岁以上患者(χ2=24.330,P<0.001);而15岁以下患者的人偏肺病毒阳性检出率与15岁以上患者比较,差异无统计学意义(χ2=0.307,P=0.579)。另外,人呼吸道合胞病毒与人偏肺病毒的阳性检出率在男女性别分布上差异无统计学意义。成功对19份人呼吸道合胞病毒和10份人偏肺病毒核酸阳性样本进行序列测定。序列比对和进化树分析结果显示:人呼吸道合胞病毒基因型别为ON1(10/19,52.63%)和BA9基因型(9/19,47.37%)。人偏肺病毒基因型别分布为B1(6/10,60.00%)、A2b型(4/10,40.00%)。结论 2015年11~12月在北京地区流感样病例中ON1、BA9基因型人呼吸道合胞病毒,B1型人偏肺病毒为主要流行株。人呼吸道合胞病毒主要感染15岁以下的患者。
【Abstract】 Objective To study the infection status and phylogenic feature of human respiratory syncytial virus and human metapeumovirus in influenza-like illness during November to December 2015 in Beijing. Methods A total of 874 influenza virus throat swab specimens were screened by real-time RT-PCR testing for human respiratory syncytial virus(HRSV)and human metapneumovirus(HMPV).Specific Ggenes were amplified and sequenced.Sequence alignments were performed and MEGA 5.0software was used for phylogenic analysis. Results The positive rate by real-time PCR screening was 2.17% and 1.60% for HRSV and HMPV,respectively.In addition,the positive rate of HRSV among subjects younger than 15 years old was higher than that of older than 15 years old(χ2=24.330,P<0.001).The positive rate of HMPV among subjects younger than 15 years old showed no significant difference from that of older than 15 years old group(χ2=0.307,P=0.579).There were no obvious differences in the positive rates of HRSV and HMPV between male and female patients.Phylogenic analysis showed that ON1-HRSV-A,BA9-HRS-B,B1-HMPV,A2b-HMPV were the prevalent genotypes. Conclusions Genotypes HRSV-ON1,HRSV-BA9 and HMPV-B1 were the predominant prevalent subtypes during the studied period in Beijing.Subjects younger than 15 years were the preponderant HRSV affected patients.
