先天性外中耳畸形听力与颞骨畸形的临床探讨

【摘要】 目的探讨先天性外中耳畸形患者的听力与颞骨畸形的相关性。方法对50例(60耳)先天性外中耳畸形患者行纯音测听、听性脑干反应(ABR)和颞骨HRCT检查,比较其听力与颞骨畸形的相关性。结果先天性外中耳畸形患者听力下降主要以传导性聋为主,平均纯音阈值为64.08±13.07 dB HL,平均气骨导差为53.28±12.05 dB HL,ABR检查结果示平均阈值为76.86±11.89dB HL,先天性外中耳畸形患者听阈与耳廓畸形分级呈正相关,与HRCT总分呈负相关。听力与听骨链、中耳发育情况和面神经发育有关,而与蜗窗关系尚待进一步研究。结论先天性外中耳畸形患者术前进行听力检查和颞骨CT检查可初步评估外耳、中耳发育程度,进而指导手术治疗。

【Abstract】 Objective To study the relationship between hearing levels and temporal bone abnormalities in patients with congenital external and middle ear malformations.Methods Pure tone audiometry,auditory brainstem responses(ABR) and CT were conducted in 50 cases(60 ears).Hearing level and temporal bone abnormalities were examined and compared.Results The pure-tone average was 64.08± 13.07 dB HL and the average A-B gap was 53.28± 12.05 dB in patients with conductive hearing loss.There was a positive correlation between hearing and Marx grading and a negative correlation between hearing and total CT scoring.Hearing was correlated with ossicular chain,middle ear development and facial nerve scorings,although it was inconclusive regarding the formation of oval/round windows.Conclusion Using pure tone audiometry and ABRs,in addition to CT,we can clinically assess hearing status and external and middle ear development,thus guiding the clinician in approaching potentially difficult surgeries.
