抑郁对慢性阻塞性肺疾病患者预后的影响

【摘要】 目的观察抑郁症对慢性阻塞性肺疾病(COPD)患者临床预后质量的影响。方法选取2013年2月2014年7月我院收治的COPD患者82例,对入选的患者应用Hamilton抑郁量表(HAMD)进行评定,筛查是否合并抑郁症。同时记录所有患者的临床资料、生活质量评估测试(CAT)问卷及圣·乔治呼吸疾病问卷(SGRQ)评分,对所有入组患者进行为期1年的随访。结果 82例被调查患者中有31例(37.80%)合并不同程度的抑郁情绪。COPD合并抑郁症组FEV1占预计值%、FEV1/FVC值低于COPD无抑郁症组,CAT评分和SGRQ评分均高于COPD无抑郁症组,差异有统计学意义(P<0.05)。随访结果显示,COPD合并抑郁症组患者1年内再入院率及病死率均高于COPD无抑郁症组,首次住院天数和1年内总住院天数均长于COPD无抑郁症组,差异有统计学意义(P<0.05)。结论COPD患者抑郁症发病率较高,抑郁对COPD患者的生活质量及预后造成不良影响。

【Abstract】 Objective To observe the effect of depression on the quality of clinical prognosis in the patients with chronic obstructive pulmonary disease(COPD). Methods 82 patients with COPD who were admitted to our hospital from February 2013 to July 2014 were selected. Hamilton Depression Rating Scale(HAMD) was used for assessment in the enrolled patients, so as to screen whether the patients were complicated with depression. Clinical data and the scores of quality of life assessment test(CAT) and St. George’s Respiratory Disease Questionnaire(SGRQ) were recorded in all patients. All enrolled patients were followed up for 1 year. Results 31 patients(37.80%) out of the 82 investigated patients were found to be complicated with different degrees of depression. The FEV1% and FEV1/FVC in the COPD with depression group were lower than those in COPD with non-depression group. CAT score and SGRQ score were higher than those in COPD with non-depression group, and the difference was statistically significant(P<0.05). The follow-up results showed that 1-year re-admission rate and mortality rate were higher in COPD with depression group than those in COPD with non-depression group, the length of stay in the first admission and the total length of stay in one year were significantly longer than those in the COPD with non-depression group, and the difference was statistically significant(P<0.05). Conclusion The incidence rate of depression in the patients with COPD is higher, and depression has an adverse effect on the quality of life and prognosis of COPD patients.
