食物不良反应患者血清中20种食物特异性IgG、IgG4的检测分析

【摘要】 目的探讨食物特异性Ig G(sIgG)和特异性IgG4(sIgG)检测在筛查食物不良反应患者问题食物的应用和临床意义。方法采用酶联免疫吸附法(ELISA)对284例食物不良反应患者血清中20种食物sIgG抗体和s IgG4抗体进行平行检测。结果食物sIgG总体阳性率55.8%,s IgG4总体阳性率44.0%,sIgG总体阳性检出率极显著高于s IgG4(P<0.01)。食物不良反应患者的sIgG和s IgG4的阳性率均明显高于健康对照组的sIgG和s IgG4(P<0.05)。小麦、大豆、花生、牛奶、鸡蛋、鳕鱼、榛子、腰果的sIgG和s IgG4检测结果的一致性较好。20种项目中,除蘑菇、花生外,其余18项食物的sIgG和s IgG4阳性率性别间差异均无统计学意义(P>0.05)。儿童组小麦、大豆、西红柿、牛奶、鸡蛋、鳕鱼、榛子、腰果和桃的sIgG阳性率均极显著高于青少年及成人组(P<0.05),小麦、鸡蛋、牛奶、鳕鱼、腰果的s IgG4阳性率均极显著高于青少年及成人组(P<0.05)。结论检测食物不良反应患者尤其是儿童血清中食物sIgG和s IgG4对于指导剔除问题食物,缓解食物不良反应症状具有较大的临床意义。

【Abstract】 Objective To investigate the clinical application and clinical significance of examination of food specific Ig G and specific IgG4 to screen the food resulting adverse food reactions. Methods SIg G and s IgG4 to 20 food antigens were measured by ELISA in serum of 284 patients of adverse food reactions. Results The total positive rates of food sIgG and s IgG4 were 55. 8% and 44. 0% respectively. The total detection rates of food sIgG were significantly higher than s IgG4. The detection rates of 20 food sIgG and s IgG4 in patients of adverse food reactions were all significantly higher than that in healthy people( P < 0. 05). The detection results of sIgG to wheat,soybean,peanut,milk,egg,codfish,hazelnut,cashewnut had good consistencies with sIgG. Except mushroom and peanut,there was no significant difference between the detection rates of sIgG,sIgG and s IgG4 to the other 18 foods( P > 0. 05). The positive rates of sIgG to wheat,soybean,tomato,milk,egg,codfish,hazelnut,cashewnut and peach in children were distinctly higher than that in elder children and alduts( P < 0. 05). The positive rates of s IgG4 to wheat,egg,milk,codfish and cashewnut in children were distinctly higher than that in elder children and alduts( P < 0. 05). Conclusion Detection of food sIgG and s IgG4 had significant clinical signification as a guidance to eliminate the harmful food and improve the symptoms of adverse food reactions.
