小牛血去蛋白提取物眼用凝胶对角膜塑形术后泪膜功能的影响
Effects of Protein-free Calf Blood Extract Eye Gel on Tear Function after Orthokeratology Treatment
【摘要】 目的:探讨小牛血去蛋白提取物眼用凝胶对早期配戴角膜塑形镜的青少年近视患者泪膜功能影响。方法:选择2015年3月至5月来我院配戴夜用型角膜塑形镜的青少年近视患者30例58眼,每日戴镜至少8 h。观察组15例28眼,戴镜后1 d至1个月给予小牛血去蛋白提取物眼用凝胶,日3次(日间)点眼。对照组15例30眼,不用小牛血去蛋白提取物眼用凝胶。分别于配戴前及配戴后1周、1个月进行主觉验光、主观症状评价(包括畏光、异物感、视疲劳、干涩感)、泪膜破裂时间、泪液基础分泌量,以及角膜上皮荧光素染色的检测。结果:戴镜后1周、1个月,观察组患者异物感较对照组轻(P<0.05);戴镜后1个月观察组患者干涩感和畏光症状较对照组轻(P<0.05),其他主观症状差异无统计学意义(P>0.05)。戴镜后1周和1个月,观察组泪膜破裂时间高于对照组(P<0.05);基础泪液分泌量比较差异无统计学意义(P>0.05);角膜上皮荧光素染色以Ⅰ级为主,观察组的角膜上皮荧光素染色级别在戴镜后各时间点均低于对照组(P<0.05)。结论:使用小牛血去蛋白提取物眼用凝胶能减轻因配戴角膜塑形镜造成的不适症状,对眼表起到保护和修复作用,并可以快速修复泪膜的不稳定状态。
【Abstract】 Objective: To investigate the effects of protein-free calf blood extract eye gel on tear function at early stage after orthokeratology treatment in young myopia. Methods: A total of 30 cases( 58 eyes) of patients accepted vision correction by orthokeratology lens from Mar 2015 to May 2015. They were divided into two groups randomly: the observation group 15 cases 28 eyes and control group 15 cases 30 eyes. After wearing lens 1 day to 1 month,the observation group was given protein-free calf blood extract eye gel three times a day,while the control group was not given protein-free calf blood extract eye gel. Inspection were carried out on the change of rational symptoms( photophobia,foreign body sensation,asthenopia,dryness of eye) and objective signs( subjective refraction,tear break-up time,Schirmer Ⅰ test) and fluorescion stain of the corneal epithelium before,1 week and 1 month after wearing the lens. Results: After wearing the lens for 1 day and 1 month,foreign body sensation in the observation group were less than that in the control group( P < 0. 05). After wearing the lens for 1 month,photophobia and dryness of eye in the observation group were less than that in the control group( P < 0. 05). After 1 day and 1 month,the tear break-up time in the observation group was lighter than that in the control group( P < 0. 05). There were no significant changes in the SchirmerⅠ test in the two group( P> 0. 05). The corneal fluorescent staining of the two group increased were mainly Grade Ⅰ,the cornea stain in the observation group was less than that in the control group during any period( P < 0. 05). Conclusions: Protein-free calf blood extract eye gel can relieve the discomfort symptoms at early stage after wearing the lens. It can stabilize the tear film during the wearing process of orthokeratology,which can protect and repair the corneal epithelium.
