双源CT容积灌注联合血管成像评估颞浅动脉-大脑中动脉搭桥术

【摘要】 目的 探讨双源CT容积灌注(CTVP)联合头颈部CT血管成像(CTA)检查对颞浅动脉-大脑中动脉搭桥术术前及术后评估的应用价值。方法 对14例一侧颈内动脉闭塞及一侧烟雾病患者于术前及颞浅动脉-大脑中动脉搭桥术术后1周分别行颅脑CTVP及头颈部CTA检查,评估手术侧颞浅动脉、桥动脉及大脑中动脉颞叶分支情况,同时观察手术侧颞顶叶血流灌注情况,评估手术疗效。结果 14例患者14根颞浅动脉均通畅,优秀12根(12/14,85.7%),良好2根(2/14,14.3%),桥血管通畅性评分为3.44±0.65,通畅率为85.7%(12/14),患侧大脑中动脉颞叶分支血管术前及术后评分分别为2.66±0.35及3.87±0.42,差异有统计学意义(P<0.05);患侧颞顶叶术前术后血流灌注数据相比,术后血容量(CBV)及血流量(CBF)增加,达峰时间(TTP)及平均通过时间(MTT)缩短,差异有统计学意义(P<0.05)。结论 双源CTVP联合CTA能了解患侧颅内外动脉,桥血管及患侧颅脑血流灌注情况,作为颞浅动脉-大脑中动脉搭桥术术前及术后的评估手段,值得应用。

【Abstract】 Objective To evaluate the value of dual source CT volume perfusion(CTVP) combined with CT angiography(CTA) in evaluation superficial temporal artery to middle cerebral artery bypass before and after operation.Methods Fourteen patients with unilateral internal carotid artery occlusion and unilateral Moyamoya who were performed superficial temporal artery to middle cerebral artery bypass before and after a week underwent both CTVP and CTA exams.The the operated superficial temporal artery,bridge artery and the branch temporosphenoid lobe of the middle cerebral artery were evaluated.At the same time the perfusion of the temporal parietal lobe was observed to evaluate the value of the operation.Results In fourteen patients,all 14 superficial temporal artery went through smoothly.12 were excellent(12/14,85.7%) while 2 better(2/14,14.3%).Bridge artery grade was evaluated 3.44±0.65 while rate was 85.7%(12/14).The grades of the sickened branch temporal parietal lobe of the middle cerebral artery were 2.66±0.35 and 3.87±0.42 before and after the operation separately.The differences were statistical significance(P<0.05).Compared with the diffusion of the sickened temporal parietal lobe before and after operation,CBV and CBF increased while TTP and MTT decreased.The differences were statistical significance(P <0.05).Conclusion Extra and intracranial arteries,bridge artery and blood perfusion were evaluated by dual source CTVP combined with CTA.It was useful to applydual source CTVP combined with CTA in evaluation superficial temporal artery to middle cerebral artery bypass before and after operation.
