成人Ph染色体阳性急性淋巴细胞白血病髓系抗原表达及临床意义
Myeloid antigen expression and clinical significance in adult Philadelphia chromosome-positive acute lymphoblastic leukemia
【摘要】 目的本研究分析成人Ph染色体阳性急性淋巴细胞白血病(Ph+ALL)髓系抗原表达情况及临床意义。方法采用流式细胞术检测93例初诊的ALL患者免疫表型,其中Ph+ALL 28例,Ph-ALL 65例,并分析其髓系抗原表达和临床意义。结果 Ph+ALL免疫表型均为B细胞,髓系抗原表达明显高于Ph-ALL组(P<0.05),其CD13、CD33的表达明显高于Ph-ALL组(P<0.05),髓系表达的抗原主要为CD13和CD33。Ph+ALL患者CD34的表达明显高于Ph-ALL组(P<0.05)。Ph+ALL组首疗程完全缓解率明显低于Ph-ALL组(P<0.05)。结论Ph+ALL患者CD13、CD33、CD34表达增高,可能与其诱导化疗首疗程完全缓解率低有关,提示未来可用于微小残留病灶(MRD)的监测,同时为潜在的靶向治疗提供理论基础。
【Abstract】 Objective This study aimed to investigate the myeloid antigen expression and clinical significance of adult Philadelphia chromosome-positive acute lymphoblastic leukemia(Ph+ALL). Methods The immunophenotype of 93 untreated ALL patients were detected by flow cytometry; Ph+ALL were 28 cases,and Ph-ALL were 65 cases. The myeloid antigen expressions and clinical significance were analyzed. Results All Ph+ALL patients expressed B cell immunophenotype. As compared with Ph-ALL group, the Ph+ALL group displayed higher expression of myeloid antigen(P<0.05), and higher expression of CD13, CD33 and CD34(P<0.05). The expressed of myeloid antigens were mainly CD13 and CD33. The CR1 rate in Ph+ALL group was lower than that of Ph-ALL group(P<0.05). Conclusion Ph+ALL patients manifested higher expression of CD13, CD33 and CD34, which may be associated with the lower induction therapy CR1 rate. It could be used for the monitoring of minimal residual disease(MRD) in the future, and provided theoretical basis for potential target therapy.
