多层螺旋CT在肺结核诊断和鉴别诊断中的应用

【摘要】 目的分析多层螺旋CT在肺结核诊断和鉴别诊断中的应用价值。方法选取医院收治的肺结核患者75例,均行多层螺旋CT及常规X线检查,根据临床病理结果分析两种检查方法的灵敏度、特异度及准确度,同时分析CT诊断的影像学特征。结果多层螺旋CT诊断肺结核的灵敏度、准确度明显高于X线检查,差异均有统计学意义(P<0.05);两种诊断方法的特异度比较差异无统计学意义(P>0.05);多层螺旋CT诊断以磨玻璃密度与实变、树芽征、小叶间增厚为主要表现,其次是空洞、随机分布的结节、支气管及血管周围间质增厚;CT对肺内隐蔽部位病变的显示率明显高于X线检查,差异有统计学意义(P<0.05)。结论多层螺旋CT诊断肺结核的灵敏度、准确度高于常规X线检查,能真实、形象地显示肺结核的各种重要征象,因此具有较高应用价值,值得在临床推广应用。

【Abstract】 Objective To explore application value of multi slice spiral CT in diagnosis and differential diagnosis of tuberculosis. Methods 75 patients with tuberculosis treated in the hospital were selected as the research objects,who underwent multi slice spiral CT and routine X-ray examination. The sensitivity,specificity and accuracy of two detection methods were analyzed according to clinical pathological results,and imaging features of CT diagnosis were analyzed. Results Sensitivity and accuracy of multi slice spiral CT in the diagnosis of tuberculosis were significantly higher than X-ray examination(P <0. 05),there was no significant difference in specificity of two diagnostic methods(P > 0. 05); main performances of multi slice spiral CT diagnosis were ground glass density and consolidation,tree in bud,interlobular incrassation,then were cavity,randomly distributed nodules,bronchus,peripheral blood vessel interstitial thickening; display rate of CT in the location of concealed lesion in lung was significantly higher than X-ray examination(P < 0. 05). Conclusion Sensitivity and accuracy of multi slice spiral CT in diagnosis of tuberculosis are higher than routine X-ray examination,can display all important signs of tuberculosis truly and vividly,therefore it has higher application value,and worth for application in clinical experience.
