【摘要】 ①目的探讨老年急性胆囊炎患者早期实施腹腔镜胆囊切除术的围手术期处理。②方法选取我院2013年1月~2015年10月收治的老年急性胆囊炎患者120例,按患者的意愿分为急诊手术组60例和早期手术组,60例。患者年龄均>60岁。急诊手术组于入院8小时内手术;早期手术组先进行围手术期处理,病情稳定后48小时之内实施手术。比较两组患者术中出血量、术后肠蠕动恢复时间、手术时间、总住院时间、住院费用及术后并发症发生率。③结果两组患者术中出血量、术后肠蠕动恢复时间差异无统计学意义(P>0.05);早期手术组患者手术时间、总住院时间、住院费用、术后并发症发生率均优于急诊手术组,差异具有统计学意义(P<0.05)。④结论合理掌握手术适应证,老年急性胆囊炎患者经积极完善的围手术期处置后早期实施腹腔镜胆囊切除术是安全、有效的。更多还原

【Abstract】 Objective To explore the experience of perioperative management of laparoscopic cholecystectomy in elderly patients with acute cholecystitis.Methods The clinical data of over 60 eldly patients with acute cholecystitis patients were selected from January 2013 to October 2015 in our hospital,60 cases of early surgery group,at the same time,the other over 60 elderly patients with acute cholecystitis patients 60 cases of emergency operation group.All of them underwent laparoscopic cholecystectomy,compared the two groups of patients with intraoperative bleeding volume,postoperative intestinal peristalsis recovery time,operation time,hospitalization time,hospitalization costs and postoperative complication rate.Results There were no significant difference between the intraoperative bleeding volume and postoperative intestinal peristalsis recovery time(P >0.05),and early operation group disease suffering from the operation time,hospitalization time,cost of hospitalization,postoperative complication rate were better than emergency surgery group,and the difference was statistically significant(P <0.05).Conclusion Reasonable surgical indications,after treatment of acute cholecystitis in elderly patients with early implementation by actively improving perioperative laparoscopic cholecystectomy is safe and effective.更多还原
