体外反搏对动脉粥样硬化性心血管疾病康复的作用
Effect of external counterpulsation on the rehabilitation of atherosclerotic cardiovascular disease
【摘要】 目的探讨增强型体外反搏的运用对动脉粥样硬化患者的康复作用。方法选取2012年12月至2014年10月在福田人民医院就诊的冠状动脉粥样硬化性心脏病(冠心病)患者107例,将他们随机分为观察组和对照组,其中观察组54例,对照组53例。对照组按常规药物治疗方案进行治疗;观察组在对照组治疗方案的基础上额外接受增强型反搏治疗,两组治疗时间均为6周,之后比较两组治疗前后患者临床症状改善情况及血清中P-选择素、血栓素B2、一氧化氮、内皮素-1及血管紧张素Ⅱ浓度的变化。结果观察组治疗6周后心绞痛改善率、硝酸甘油(NTG)用量减少率、运动耐量提高率及总有效率均显著高于对照组,差异有统计学意义(P<0.01);治疗前,两组血清P-选择素、血栓素B2、血管紧张素Ⅱ及内皮素-1浓度比较,差异无统计学意义(P>0.05);观察组上述4种血清学指标治疗后与治疗前比较均有显著改善,差异有统计学意义(P<0.05);对照组上述4种血清学指标治疗后与治疗前比较,差异均无统计学意义(P>0.05);观察组治疗后的血清学指标与对照组治疗后的相应血清学指标比较均有显著改善,差异有统计学意义(P<0.05)。结论体外反搏治疗能显著提高冠心病患者的临床疗效,且能显著降低患者血清中缩血管物质水平,该方法值得在临床推广应用。
【Abstract】 Objectives To investigate the effect of enhanced external counterpulsation on the rehabilitation of patients with atherosclerosis.Methods From December 2012 to October 2014,107 patients with coronary heart disease were randomly divided into observation group and control group:54 cases in observation group and 53 cases in control group.Control group received routine drug treatment,and observation group received enhanced anti-stroke treatment based on routine drug treatment.The two groups were treated for 6 weeks.Rate of clinical symptom improvement,serum concentrations of P-selectin,thromboxane B2(TXB2),nitric oxide(NO),endothelin-1(ET-1) and angiotensin Ⅱ(Ang Ⅱ) of the two groups were compared.Results In 6 weeks of treatment,rates of angina improvement,reduction dosage in niter acid gansu oil(NTG),exercise tolerance improvement and total effectiveness of observation group were significantly higher than those of control group(P<0.01).Before treatment,serum concentrations of P-selectin,TXB2,ET-1 and Ang Ⅱ of the two groups had no significant difference(P>0.05).Serum concentrations of P-selectin,TXB2,ET-1 and Ang Ⅱ of observation group after treatment had significantly improved(P<0.05);but they did not significantly improved in control group after treatment(P>0.05).Serum concentrations of P-selectin,TXB2,ET-1 and Ang Ⅱ of observation group after treatment significantly improved when compared with those of control group after treatment(P<0.05).Conclusions In vitro anti-pulse therapy can significantly improve the clinical efficacy in patients with coronary heart disease and reduce the serum concentrations of vasoexcitor materials.It is worthy of popularization and application.
