【摘要】 目的探讨快速康复外科技术(fast track surgery,FTS)在腹腔镜子宫全切术围术期应用的临床效果。方法选取75例行FTS腹腔镜子宫全切术患者以及78例行传统模式腹腔镜子宫全切术患者。比较两组手术结束距拔除气管导管的时间、术后首次排气的时间、术后并发症的发生率以及术后3、6、12、24h疼痛视觉模拟评分(VAS)。同时比较两组间术后平均住院时间和平均住院费用。结果 FTS模式组较传统模式组拔除气管导管的时间、术后首次排气的时间显著缩短(P<0.05)。两组术后并发症的发生率差异无统计学意义(P>0.05)。术后3h两组VAS评分差异无统计学意义,FTS组6、12、24hVAS评分均小于传统模式组。差异有统计学意义(P<0.05)。FTS组较传统模式组平均术后住院时间缩短,平均住院费用降低。差异有统计学意义(P<0.05)。结论快速康复技术用于腹腔镜子宫全切术围术期是安全有效的,值得深入推广。更多还原

【Abstract】 Objective To evaluate the safety and efficacy of fast track surgery in patients undergoing total laparoscopic hysterectomy.Methods 153 patients with benign gynecologic tumor undergoing total laparoscopic hysterectomy were divided into two groups:fast track surgery program(n= 75,received perioperative FTS program)and conventional care group(n = 78,received the traditional perioperative program).The time of withdraw tracheal intubation,first flatus time, postoperative complications and postoperative VAS score at 3hours,6hours,12 hours and 24 hours,postoperative hospital stay,hospitalization expenditure were compared between the two groups.Results FTS program group was associated with a significantly shorter in time of withdraw tracheal intubation and first flatus time(P<0.05),VAS score at 6hours,12 hours and 24 hours was lower in FTS program group compared with conventional care group(P<0.05).There were no differences in postoperative complications between the two groups(P>0.05).Fast track program was associated with a significantly shorter postoperative hospital stay,lower hospitalization expenditure(P<0.05).Conclusion Fast track surgery is feasible and effective in benign gynecologic tumor undergoing total laparoscopic hysterectomy.更多还原
