小儿轮状病毒肠炎血清与粪细胞因子变化及儿泻停颗粒干预的影响
Changes of cytokines in sera and stool of rotavirus enteritis infantile and intervention of Er-Xie-Ting Granule
【摘要】 [目的]探讨小儿轮状病毒肠炎(IRVE)血清、粪细胞因子变化与儿泻停颗粒干预的影响。[方法]将112例6个月~3岁的患儿随机分为观察组和对照组各56例。2组均予以常规治疗。观察组采用儿泻停颗粒1~6个月0.5g/次;7个月~2岁1.0g/次;3岁2.0g/次;3次/d口服,3d一疗程。对照组采用布拉氏酵母菌散剂<1岁,0.125g/次;1~3岁0.25g/次,2次/d口服。另设40例健康体检儿童为健康对照组。治疗前3组取静脉血及粪,采用EILAIS法测定血粪IFN-γ及TNF-α水平,观察组与对照组在治疗3d后再次测定IFN-γ,TNF-α。[结果]观察组显效率与总有效率明显高于对照组(P<0.05,P<0.01),差异有统计学意义。治疗前2组血清与粪IFN-γ、TNF-α水平比较(P>0.05)。但与健康组比较(P<0.01)差异有统计学意义。2组治疗后血清与粪IFN-γ水平较治疗前明显上升(P<0.01);TNF-α较治疗前明显下降(P<0.01);且观察组较对照组更明显(P<0.01)差异有统计学意义。但仍然未能达健康组水平。[结论]IRVE与血清和粪炎症细胞因子失调有关。儿泻停颗粒治疗IRVE临床疗效可能与其有效调控IFN-γ、TNF-α的水平,提高抗病毒及发挥免疫保护功能有关。
【Abstract】 [Objective]To investigate the changes of sera,stool cytokines in children with rotavirus enteritis(infantile rotavirus enteritis,IRVE)and the intervention of Er-Xie-Ting Granule(EXTG).[Methods]One hundred and twelve cases aged from 6months to 3years were randomly divided into observation group and control group(n=56in each).Two groups received conventional treatment.The treatment group was given EXTG three times per day for three days,and the dosage was 0.5g per times for 1~6-month old children;1.0g per times for 7-month to 2-year old children;2.0g per times for 3-year old children.Control group was given Saccharomyces boulardii Sachets 0.125 g per times for 1-year old children,0.25 g per times for 1~3-year old children,2times per day for three days.Additionally,40 healthy cases were set as healthy controls.Venous blood and stool were collected for determination of sera and stool IFN-gamma and TNF alpha level with ELISA before treatment in three groups,and the parameters were checked again in the observation group and control group after treatment for 3days.[Results]The total effective rate was obviously higher in observation group than that in control group with statistical significance(P<0.01).Before treatment,sera IFN-gamma,TNF alpha levels were much higher in the observation group and control group than that in the healthy control group respectively(P<0.05,P<0.01).After treatment,sera and stool IFN-gamma levels increased significantly(P<0.01),while that of TNF alpha decreased significantly(P<0.01),and the changes was more obvious in observation group than in control group(P<0.01).[Conclusion]IRVE is associated with the disorder of sera and stool inflammatory cytokines.EXTG was effective for the treatment of IRVE,which might be associated with the regulation of IFN-gamma and TNF alpha.
