广州市番禺区9989例适龄妇女宫颈癌人乳头瘤病毒筛查结果分析
Analysis on screening results of human papillomavirus in 9989 cervical cancer women of the right age in Panyu district,Guangzhou
【摘要】 目的分析人乳头瘤病毒(HPV)基因型在广州市番禺区不同年龄段的户籍妇女中的感染分布情况及其与其他地区人群的差异性,并探讨HPV在妇女宫颈病变中的型别分布特征。方法广州市番禺区内9 989例妇女采用基质辅助激光解析电离飞行时间质谱技术(MALDI-TOF-MS)对其宫颈脱落细胞进行HPV检测分析,并对HPV阳性人群进行液基细胞学检测(TCT)及阴道镜活体组织检查(活检)。结果广州市番禺区989例妇女HPV总感染率为8.33%,HPV感染在各个年龄段中检出率差异无统计学意义(χ2=4.83,P>0.05)。HPV感染以HPV 52、16、58、51为主要感染型别,且复合感染随着病变程度的增加而增加。HPV感染者伴TCT结果为意义不明的非典型鳞状上皮细胞(ASC-US)程度以上的受检者行阴道镜检查及活检92例,发现1例原位癌患者HPV感染类型为二重感染,感染型别为HPV 31及33,癌前病变及癌症的发病率为0.17%。结论HPV在年龄的感染率分布与其他地区的分布一致呈U型分布,广州市番禺区妇女的宫颈癌前病变及癌症发病率远低于其农村妇女。
【Abstract】 Objective To analyze the distribution of human papillomavirus( HPV) genotypes infection in resident women of different age groups and the difference compared with the population in other districts,explore the distribution characteristics of HPV genotypes in female cervical lesions. Methods HPV detection and analysis in cervical exfoliated cells were conducted by matrix assisted laser resolution mass spectrometry( MALDI- TOF- MS) technology among 9 989 women in Panyu district,Guangzhou; liquid- based cytology test( TCT)and colposcopic biopsy were carried out among the HPV positive population. Results The total infection rate of HPV was 8. 33%,there was no statistically significant difference in the detection rate of HPV infection among the women of different age groups( χ2= 4. 83,P > 0. 05).HPV 52,HPV 16,HPV 58,and HPV 51 were main HPV infection genotypes,the incidence rate of mixed infection increased with aggravation of the diseases. Ninety- two women were diagnosed as HPV infection combined with above ASC- US,then colposcopic examination and biopsy were performed,one woman with carcinoma in situ of cervix was found with HPV double infection,the genotypes were HPV 31 and HPV 33; the incidence rate of cervical precancerous lesion and cervical cancer was 0. 17%. Conclusion The infection rate of HPV shows " U"- type distribution,which is the same as the infection rates of HPV in the other areas,the incidence rate of cervical precancerous lesion and cervical cancer among the women in Panyu district of Guangzhou city is significantly lower than that among rural women.
