【摘要】 目的分析食管梭形细胞癌的临床病理特点。方法回顾性分析7例食管梭形细胞癌的临床和病理资料。结果 7例食管梭形细胞癌均为腔内型息肉样肿块,组织学示6例癌成分为鳞状细胞癌,1例为基底细胞样鳞状细胞癌;肉瘤成分多样分化,3例为平滑肌肉瘤,3例为未分化肉瘤,1例为黏液纤维肉瘤。术前确诊率为14.3%。病理分期:T1期4例,T2期2例,T3期1例。2例出现淋巴结转移,均为癌成分转移。结论食管梭形细胞癌术前确诊率低,淋巴结转移率相对也较低,癌成分在其淋巴结转移中常见,外侵不明显。手术切除是首选治疗方案。更多还原

【Abstract】 Objective To investigate the clinical characteristics of esophageal spindle cell carcinoma( carcinosarcoma).Methods The clinical data of 7 patients treated by surgery and pathologically diagnosed as esophageal spindle cell carcinoma between January 2006 and January 2013 were retrospectively reviewed. Results The 7 patients were diagnosed as esophageal carcinosarcoma,all of them were polypoid type,and preoperative diagnosis rate was 14. 3%.There were 6 male and 1 female patients aged from 49 to 80 years with a median age of 58 year. 6 cases of cancer component were squamous cell carcinoma,one case was basaloid squamous cell carcinoma. Sarcoma component had diverse differentiation( 3 cases of leiomyosarcoma,3 malignant fibrous histiocytoma,and 1 myxofibrosarcoma). The involvement of local lymph nodes was present in 2 patients,with an incidence of 28. 6%. including metastatic carcinoma alone in 2patients. According to the international TNM staging system for esophageal carcinoma,4 patients were in stage I,2 patients in stageⅡ,and 1 patient in stage Ⅲ. Conclusions The preoperative diagnosis of esophageal carcinosarcoma is low.Curative resection of this tumor may achieve good prognosis because of its lower incidence of lymph node metastasis and less invasive tendency,while the carcinoma more frequently appears in local lymph node metastasis. About the histogenesis of esophageal carcinosarcoma remains to be further studied.更多还原
