不同吸痰深度对慢性阻塞性肺疾病患者血氧饱和度的影响

【摘要】 目的:探讨不同吸痰深度对慢性阻塞性肺疾病(COPD)患者血氧饱和度的影响。方法:将66例不能自主完成咳嗽排痰、不能自主变动体位、无建立人工气道的COPD患者随机分为实验组和对照组各33例,实验组采用浅度吸痰法,对照组采用深度吸痰法;将两组吸痰后的经皮脉搏血氧饱和度(Sp O2)与吸痰前的Sp O2、吸痰后Sp O2下降幅度、吸痰后Sp O2恢复时间及达稳定时间、痰鸣音变化、安全性指标比较进行对比。结果:两组不同深度吸痰后Sp O2、吸痰后Sp O2下降幅度、吸痰后Sp O2恢复时间及达稳定时间比较差异有统计学意义(P<0.01)。结论:采取浅度吸痰法,可有效帮助患者排痰,对COPD患者血氧饱和度影响小,对气道黏膜损伤小,一次插管成功率高,较少引起并发症,是一种合理、可取的吸痰深度。

【Abstract】 Objective: To discuss the influence of the different suction depth on blood oxygen saturation of patients with chronic obstructive pulmonary disease( COPD). Methods: 66 COPD patients who could not have self- cough expectoration and change their body position by themselves and did not have the establishment of artificial airway were randomly divided into the experimental group and the control group( 33 cases in each group). The shallow suction method was used in the experimental group and the deep suction method was used in the control group,Sp O2 before and after suction,the decline of Sp O2 and the time of Sp O2 return to normal and to stability after suction,change of wheezy phlegm by auscultation and other safety indexes were compared between the two groups. Results: There were statistically significant differences in the comparison of Sp O2 and the decline of Sp O2 after suction in different depth,the time of Sp O2 return to normal and to stability after suction between the two groups( P < 0. 01). Conclusion: Application of the shallow suction method can effectively help the patients with expectoration,and it has little influence on Sp O2 of COPD patients,little injury of airway mucosa and less complication.
