【摘要】 目的:观察慢性心力衰竭(CHF)患者接受心脏再同步化治疗(CRT)的临床效果。方法:回顾性分析2008年1月至2013年12月我科收治的46例CHF患者需要置入CRT的临床资料,比较CRT置入术前和术后6个月的心功能分级(NYHA)、左室射血分数(LVEF),左室舒张内径((LVED)、心电图QRS波群时限、6 min步行距离。结果:患者根据新器械置入和原有起搏器升级分为新置入组(A组,n=23)和起搏器升级组(B组,n=23),A、B组CRT后反应良好率分别为69.6%(16/23)和73.9%(17/23),无明显差异(P>0.05);与治疗前相比,两组的NYHA、LVEF均有明显改善,LVED明显缩小、QRS时限明显缩短,6 min步行距离显著提高,P均<0.05。结论:CRT能改善药物治疗不满意的CHF患者的心功能不全症状。更多还原

【Abstract】 Objective :To observe the clinical effect of patients with chronic heart failure(CHF)treated by cardiac resynchronization therapy(CRT). Methods : The clinical data of 46 patients with CHF treated by CRT were analyzed retrospectively from Jan 2008 to Dec 2013. The cardiac function(NYHA), LVEF,LVED,QRS duration,6- minute hall walk distances were compared before and after CRT implantation 6 months. Results : The cases were classified as new implants group(group A,n =23) and upgradings group(group B,n =23) according to the new implants. The effectiveness of CRT were no significant different between the group A and group B [69.6%(16 / 23) vs 73.9%(17 /23),P >0.05 ]. Compared with the before treatment,the NYHA、LVEF were significant improvement, the LVED 、QRS duration significantly decreased and the 6-minute hall wall distances significantly improved,all P <0.05. Conclusion :CRT can obviously improve the symptoms of cardiac dysfunction of patients with CHF.更多还原
