S基因进化树分析证实乙型肝炎病毒存在性传播途径

【摘要】 目的从分子水平证实乙型肝炎病毒(HBV)存在性传播途径。方法本研究共收集36对婚检时女方乙型肝炎病毒表面抗原(HBs Ag)阳性而男方HBs Ag阴性的夫妻,随访4年后其中10对夫妻中男方HBs Ag变为阳性,此10对夫妻作为病例组,随机选择10名无关男性HBV感染者与病例组中女方组成对照组。提取所有实验对象血清中的HBV-DNA,应用巢式PCR扩增HBV S基因,并用高通量测法对HBV S基因进行测序。分别对病例组、对照组中男、女性双方HBV S基因进行序列比对,对所有30个HBV S基因序列进行基因进化树分析。结果病例组S基因序列相似性高于对照组,差异具有统计学意义(P<0.05)。S基因进化树分析显示10对夫妻中,5对夫妻间进化距离明显短于女方与无关男性HBV感染者间进化距离。2对夫妻间进化距离和女方与无关男性HBV感染者间进化距离基本相同,3对夫妻间进化距离长于女方与无关男性HBV感染者间进化距离。结论 HBV存在性传播途径,可经性途径由女性传染给男性。

【Abstract】 Objective To confirm the existence of sexual transmission of hepatitis B virus(HBV) from the molecular level.Methods 36 couples of HBs Ag positive women and their HBs Ag negative spouses were collected. After 4 years follow-up visit, 10 couples which Hbs Ag negative men transformed to positive as case group, and randomly selected 10 unrelated HBs Ag positive men and the 10 HBs Ag positive women as control group. HBV-DNA of all subjects’ sera were extracted, the HBV-S gene was amplificated by Nest-PCR, and sequenced by high throughput technology. The HBV S gene sequences of males and females from case and control groups were compared respectively, and phylogenetic tree analyzed all 30 HBV S gene sequence. Results The sequence similarity of S gene in case group was higher than controls, and the difference was statistically significant(P<0.05). The phylogenetic tree analysis of S gene showed that evolutionary distances between husbands and wives of 5 couples were obviously shorter than those wives and unrelated males infected with HBV, 2 couples’ was equal, and 3 couples’ was longer. Conclusion Sexual transmission of HBV was existence, and males could be infected with HBV from females through Sexual contalt.
