【摘要】 目的调查分析某医院4年间血流感染中产ESBLs肺炎克雷伯菌的分离率和对常用抗生素的耐药率。方法细菌鉴定采用VITEK 2 Compact全自动微生物鉴定仪方法;ESBLs检测采用筛选和确认的方法;药敏试验采用MIC法和K-B法。结果 4年间血流感染共分离肺炎克雷伯菌6640株,产ESBLs的为5486株,占82.5%。四个年度的产酶率分别为65.9%,75.3%,90.4%,95.8%。对碳青霉烯类和四代头孢耐药率为零,对1~3代头孢和磺胺类几乎全耐药;对喹诺酮、氨基糖苷类耐药率也较高。结论四年间,产ESBLs肺炎克雷伯菌在血流感染中无论是分离率还是耐药率都是逐年上升,正确合理的使用抗生素,对控制产ESBLs肺炎克雷伯菌传播显得尤为重要。更多还原

【Abstract】 Objective To investigate and analyze the separation rate and the prevalence of antibiotic resistance of ESBLs pneumonia klebsiella in bloodstream infections in a hospital in four years. Methods Bacteria identification using Vitek2 Compact automatic Microbe instrument; ESBLs test using the method of screening and confirmation; Drug sensitive test using the MIC and K- B method. Results Totally 6640 strains pneumonia klebsiella were obtained in 4 years; 5486 strains producing ESBLs,Accounted for 82. 5%; Enzyme production rate were 65. 9%,75. 3%,90. 4%,95. 8% respectively in 4 years; The resistant rate is zero to carbon penicillium alkene and four generation cephalosporin; They were almost all resistant to 1- 3 generation cephalosporins and sulfa. The resistant rate were higher for quinolone and aminoglycoside.
