【摘要】 目的探讨食管癌脑转移的临床特征、治疗方法与预后因素。方法回顾性分析2009年7月至2013年7月我院收治的6例食管癌脑转移患者的临床特征、治疗方法及预后相关因素,并结合国内外文献进行分析。结果发生脑转移的6例食管癌患者中,男性4例,女性2例。分期均为III-IV期。单发脑转移者2例,多发脑转移者4例。6例患者均接受颅脑放疗或放化疗治疗。全组平均生存时间为16个月,2例单纯脑转移不伴有其他脏器远处转移的患者生存期17.5个月,颅内多发转移瘤的患者生存期7-18个月。结论食管癌脑转移以男性多见,与分期及HER-2过表达相关,治疗上应采用以手术切除联合放疗为主的综合治疗。远处淋巴结转移、颅外转移及患者年龄可能为预后相关因素。更多还原

【Abstract】 Objective To explore the clinical features,therapeutic approach and prognostic factors for esophageal carcinoma with brain metastases.Methods The records of six patients with brain metastases from esophageal carcinoma who were treated between July 2009 and July 2013 were retrospective analyzed and the related literature was reviewed.Results Of the six patients,four cases were male,two cases were female;they are all stage III-IV.Two cases have single brain metastae and four cases have multiple brain metastaes.They all accept radiotherapy or chemoradiotherapy.The average survival for all patients was16 months,and the survival in single brain lesion and multiple brain metastaes was 17,5months and 7-18 months respectively.Conclusion Brain metastases from esophageal carcinoma tended to occur in male patients,and they are related to cliclnal stages and overexpression of HER-2.Operation resection combined with radiotherapy is suggested.The distant lymph node metastasis,extracranial metastasis and the patient’s age maybe are prognostic factors.更多还原
