单向式全胸腔镜肺癌根治术探讨

【摘要】 目的探讨单向式胸腔镜肺叶切除术治疗非小细胞肺癌安全性、有效性和可行性。方法回顾性分析82例非小细胞肺癌患者施行单向式全胸腔镜肺叶切除术的临床资料。结果成功完成单向式全胸腔镜肺癌根治术79例;3例因胸腔镜下难以控制出血,增加10 cm辅助小切口。术中清扫淋巴结数量为7~22枚,手术时间80~210 min,术中出血量50~600 ml。术后胸管引流时间3~8 d,术后住院时间4~9 d。无围手术期死亡,无肺动脉栓塞等严重并发症。结论单向式全胸腔镜肺叶切除术具有创伤小、恢复快、疼痛轻、住院时间短等优点,淋巴结清扫彻底,是一种安全有效的手术方式,是治疗早期肺癌的可靠方法。尤其对初学者易操作、易掌握。

【Abstract】 Objective To discussion the safety,effectiveness and feasibility of single-direction complete video-assisted thoracoscopic lobectomy in the treatment of non-small cell lung cancer. Methods We reviewed the clinical data of 82 patients with non-small cell lung cancer received single-direction complete video-assisted thoracoscopic lobectomy. Results 79 patients were successfully subjected to singledirection complete video-assisted thoracoscopic lobectomy,3 patients were increased 10 cm incision for hemostasis. The number of clean-up lymph node was 7-22,operation time was 80-210 min,surgical blood loss was 50-600 ml,chest tube placement time was 3-8 d,post-operative hospitalization days was 4-9 d,no perioperative death,no serious complications such as pulmonary embolism. Conclusion Single-direction complete video-assisted thoracoscopic lobectomy in the treatment of non-small cell lung cancer is afety,effectiveness and feasibility, this operation can reduce trauma and pain, promote recovery, shorter hospitalization time,complete mediastinal and hilar lymph node dissection. The operation is easy to study and operate for beginners.
