颞浅动脉和上颌动脉颅外段的超声检测及其在烟雾病诊疗中的应用价值

【摘要】 目的探讨彩色多普勒超声检测颞浅动脉和上颌动脉颅外段的可行性,并且评估该方法在烟雾病诊疗中的应用价值。方法 24例健康志愿者及38例烟雾病患者分别测量颞浅、上颌动脉的收缩期峰值流速(PSV)、舒张末期流速(EDV)及阻力指数(RI),获得正常人群参考值,比较烟雾病患者术前1周内和术后10 d内上述血管的血流动力学变化,同时比较烟雾病患者术前上述血管的血流动力学参数与健康志愿者之间是否存在差异。结果从24例健康志愿者中获得了颞浅、上颌动脉血流动力学参数的正常参考值;烟雾病患者在术前颞浅、上颌动脉的RI就低于健康志愿者;术后10 d与术前1周比较,颞浅、上颌动脉的流速显著增高、RI显著降低。结论彩色多普勒超声检测颞浅、上颌动脉颅外段是切实可行的,该技术能作为烟雾病诊断及手术疗效评估的有力工具。

【Abstract】 Objective To investigate the feasibility of color Doppler ultrasound to detect superficial temporal artery and extracranial maxillary artery and to assess the value of this method in the diagnosis and surgery treatment of moyamoya disease.Methods 24 healthy volunteers and 38 patients with moyamoya disease were enrolled.The peak systolic velocity(PSV),end diastolic velocity(EDV) and resistance index(RI) of their superficial temporal artery and maxillary artery were measured.Normal reference values are calculated.The vascular hemodynamic changes between one week before and 10 days after surgery were compared.The vascular hemodynamic difference between moyamoya disease patients and healthy volunteers were also compared.Results Normal reference value of superficial temporal artery and maxillary artery blood flow parameters were obtained from 24 healthy volunteers.The RI of superficial temporal artery and maxillary artery of Moyamoya disease patients before surgery were already lower than that of healthy volunteers;The superficial temporal artery and maxillary artery flow velocity were increased with RI decreased significantly 10 days after surgery.Conclusions The color Doppler ultrasound detection of superficial temporal artery and extracranial maxillary artery is feasible.This technique can be used as a powerful tool of moyamoya disease diagnosis and surgery effectiveness assessment.
