辛伐他汀与布地奈德对慢性阻塞性肺疾病后患者血细胞因子与肺功能的影响研究

【摘要】 目的:探讨辛伐他汀与布地奈德治疗慢性阻塞性肺疾病后患者血细胞因子与肺功能的变化。方法:随机选择慢性阻塞性肺疾病患者60例,按数字随机法分为观察组30例和对照组30例,对照组按照诊疗常规给予治疗,观察组在常规的治疗基础上加用辛伐他汀与布地奈德治疗。两组均与入院第1天及治疗后2周后进行血细胞因子(IL-17、IL-33、IFN-r)与肺功能(FEV1%、FEV1/FVC)检查。比较其治疗前后IL-17、IL-33、IFN-r和肺功能FEV1%、FEV1/FVC。结果:治疗前两组血细胞因子和肺功能无明显差异(P>0.05),对照组治疗2周后与入院前比各项指标无明显变化(P>0.05)。观察组治疗2周后IL-17、IL-33明显降低,肺功能FEV1%和FEV1/FVC较治疗前显著升高(P均<0.05),且显著高于对照组(P均<0.01)。血细胞因子与肺功能变化呈负相关,但无统计学意义(P>0.05)。结论:IL-17、IL-33水平在COPD患者体内明显增高,COPD患者由于气流受限的原因FEV1%、FEV1/FVC会比正常人低,使用辛伐他汀与布地奈德治疗后IL-17、IL-33水平明显降低,FEV1%、FEV1/FVC指标较治疗前明显增高。辛伐他汀与布地奈德治疗慢性阻塞性肺疾病值得临床推广使用。

【Abstract】 Objective:To investigate the effects of simvastatin on pulmonary function and vascular endothelial growth factor(IL-17,IL-33,IFN-r)levels in induced sputum of patients with chronic obstructive pulmonary disease(COPD).Methods:60 patients with COPD were selected and divided into observation group(n=30)and control group(n=30)randomly.Serum VEGF levels in induced sputum were detected by ELISA on the first day and after two weeks treatment in hospital,respectively.Meanwhile,the pulmonary function measurements were performed.Results:There were no significant differences at the pulmonary function(FEV1%and FEV1/FVC)and VEGF levels in induced sputum between the two groups before treatment(P>0.05).The RT group showed no significant changes in any parameters before and after two weeks treatment(P>0.05).FEV1%,FEV1/FVC and VEGF levels in induced sputum in the RST group after two weeks treatment significantly increased compared with those before treatment and the RT group(P<0.01,P<0.01,P<0.05).But there were no significant differences in serum IL-17,IL-33,IFN-r levels between the two groups before and after two week treatment.The IL-17,IL-33,IFN-r levels in induced sputum were positively correlated to FEV1%and FEV1/FVC after two weeks treatment(P<0.05).Conclusion:Simvastatin may reduce the decline in pulmonary function and decrease the levels of IL-17,IL-33,IFN-r in induced sputum of patients with COPD.Improvement in pulmonary function may be related to down-expression of lung.
