腹部肾上腺外副神经节瘤的影像诊断
Imaging Findings of Abdomen Extra-adrenal Paraganglioma
【摘要】 目的:分析腹部肾上腺外副神经节瘤的CT和MRI表现,探讨其影像学特点。方法:回顾性分析15例经手术病理证实的腹部肾上腺外副神经节瘤的影像表现。结果:腹膜后10例,CT平扫呈稍低密度,内均可见囊变、坏死,3例呈渐进性延迟强化,7例呈动脉期中度/明显强化,高峰位于动脉期或门脉期,延迟期缓慢下降。膀胱5例,CT/MRI平扫均呈均匀等密度/稍长T1等T2信号,动脉期明显强化,门脉期及延迟期缓慢下降。所有病例均未见淋巴结转移。结论:中老年人腹膜后占位,囊变、坏死明显,呈渐进性延迟强化或快/慢进慢出型,应考虑肾上腺外副节瘤的诊断;膀胱副节瘤多较小,密度/信号较均匀,T2WI呈等信号以及"快进慢出型"、明显强化的特点有助于该病的诊断及鉴别诊断。
【Abstract】 Objective: To explore the imaging features of abdomen extra- adrenal paraganglioma and to improve the imaging diagnostic abilities. Methods: The CT and MRI findings of 15 patients pathologically confirmed abdomen extra- adrenal paraganglioma were retrospectively studied and compared with pathological features. Results: The leisions of 10 retroperitoneal cases presented as slightly low attenuation,with different scope of intratumoral cystic degeneration or necrosis,3 of them showed"progressive delayed enhancement"and 7 of them showed medium /obvious enhance in artery phase( The peak of enhancement was in the artery or portal phase),then enhanced degree dropped slowly in the delay phase. The 5 urocystic cases manifested homogeneous isodensity on the CT plain scan and slightly hypointensity on T1 WI and isointensity on T2 WI,they enhanced obviously in the artery phase then drop slowly in the portal and delay phase. None had lymphatic metastasis. Conclusions: The disease should be considered for abdomen extra- adrenal paraganglioma,combined with a solid tumor mixing with conspicuous cyctic degeneration or necrotic area,located in the retroperitoneum,in a middle or old- aged patient,with a morderate or obvious enhancement,especially,showed "progressive delay enhance"or"fast/slow in and slow out"enhancement mode. The paraganglioma in urinary bladder is often small,but the characteristic signs of homogeneous attenuation or signal,isointensity on the T2 WI,the enhancement mode of"fast in slow out"and enhanced obviously are helpful for diagnosis and differential diagnosis.
