【摘要】 目的探讨肿瘤患者合并侵袭性肺曲霉菌病的临床状况和预后危险因素,对该病预防与治疗提供参考依据。方法回顾性分析2008年7月1日-2011年6月30日确诊和临床诊断为侵袭性肺曲霉菌病的肿瘤患者临床资料;根据预后将患者分为死亡组和存活组,应用SPSS11.5软件进行预后危险因素分析。结果肿瘤患者中肺癌患者占51.19%,多数患者有咳嗽、咳痰、发热等症状和肺部体征;死亡组36例,存活组48例,单因素方差分析显示,诊断前30d内接受过放化疗、血白细胞减少和拒绝抗曲霉菌属药物治疗与预后有关,logistic回归模型进行多因素分析示患者在诊断前30d内接受过放化疗和拒绝抗曲霉菌属药物治疗为独立的死亡危险因素。结论侵袭性肺曲霉菌病的肿瘤患者死亡率高,早期发现诊断、避免放化疗和及时应用抗曲霉菌属药物治疗至关重要。更多还原

【Abstract】 OBJECTIVE To investigate the clinical features and risk factors for prognosis of invasive pulmonary aspergillosis(IPA) in tumor patients so as to prevent and treat the disease.METHODS We analyzed the clinical feature of 84 cases of proven or probable IPA from July 1st 2008 to June 30th 2010 in the hospital retrospectively.According to the prognosis the patients were divided into the death group and the presence group.SPSS 11.5 software was employed to analyze the risk factors for the prognosis.RESULTS Totally 51.19% of the tumor patients were lung caner patients,the cough,expectoration,fever and lung signs were the main clinical features.36 cases were dead and 48 cases were survival.Single factor analysis of variance indicated that diagnosis before 30 days of receiving radiotherapy and chemotherapy,leucopenia,and refusing the antiaspergillus drug treatment were associated with the prognosis.logistic regression model for multivariate analysis indicated that the patients with receiving radiotherapy and chemotherapy,and refusing the antiaspergillus drug treatment were the independent risk factors for the death.CONCLUSION Tumor patients complicated with IPA are with high mortality,thus the detection and diagnosis in the early stage,avoiding the radiotherapy and chemotherapy,and the application of antiaspergillus drugs in a timely manner are crucial.更多还原
