【摘要】 目的:总结妊娠期肾病综合征的临床特点,对围生结局进行分析,为临床诊治提供参考。方法:对2008年1月～2012年9月收治的240例重度子痫前期的临床资料进行回顾性分析,65例伴有肾病综合征纳入肾病组,175例重度子痫前期患者纳入对照组,对两组患者的临床指标、相关生化指标进行对比分析。结果:肾病组多为初产妇,血压升高以舒张压升高为主,发病孕周和终止妊娠孕周早于对照组。肾病组的肾功能损害较严重,其血清总蛋白、白蛋白低于对照组,24h尿蛋白、尿素氮及肌酐高于对照组。肾病组新生儿窒息率、围产儿死亡率高于对照组,新生儿出生体重低于对照组。结论:妊娠期肾病综合征病情较重且进展快,以初产妇多见同时发病孕周较早。妊娠期肾病综合征对母婴影响较大,在综合治疗的基础上适时终止妊娠,以确保母婴安全并减少并发症。更多还原

【Abstract】 Objective:To summarize the clinical characteristic of nephritic syndrome of pregnancy(NSP) and analyze the perinatal outcomes for the clinical treatment.Methods:240 pregnant women with severe preeclampsia who were hospitalized in our hospital from January 2008 to September 2012 were retrospectively analyzed.The patients were divided into two groups: 65 cases with nephritic syndrome as NSP group and 175 cases without nephritic syndrome as control group.To compare and analyze the clinical indexs and correlated biochemical indexes of the two groups.Results:NSP was more likely to happen in primipara with severely increased diastolic pressure.The occurrence and termination weeks in NSP group were significantly earlier than those of control group.The maternal serum total protein and albumin in NSP group were lower than those of control group.24 h urine protein,serum lactic dehydrogenises and creatinine in NSP group were higher than those of control group.And renal function lesion was also relatively heavier in NSP group.Neonate asphyxia rate and the mortality of perinatal fetal in NSP group were higher than those of control group.Brith weight in NSP group was lower than that of control group.Conclusion:NSP which is more likely to happen early in primipara has severe condition and quick development.NSP has large effects on mother and infants.Termination of pregnancy in time should be done based on the multipurpose therapy to reduce the complication and ensure the safety for mother and baby.更多还原
