【摘要】 目的探讨胸腰段脊柱爆裂性骨折经伤椎椎弓根植骨、并行椎弓根螺钉固定的治疗效果。方法 2008年1月~2011年12月对87例胸腰段爆裂性骨折的患者采用经伤椎椎弓根植骨并椎弓根螺钉固定治疗,手术前后随访观测VAS评分、后凸角度、并发症及神经功能恢复情况。结果术后随访12~48个月,平均(25.3±12.5)个月,无内固定失败的病例及严重并发症发生。大部分患者的神经功能较术前均有1～2级的改善。术后与末次随访时的VAS评分分别为(5.3±0.7)分与(2.8±0.5)分,显著低于术前(P<0.05);术后与末次随访时的后凸角度分别为(-3.1±6.2)°与(0.7±0.4)°,显著低于术前(P<0.05)。结论经伤椎椎弓根植骨并椎弓根螺钉固定治疗伤椎胸腰段脊柱爆裂性骨折安全可靠,并发症少,是一种有效的治疗方法。更多还原

【Abstract】 Objective To explore the clinical efficacy of transpedicular intracorporeal grafting combined with pedicle screw fixation for treatment of thoracolumbar burst fractures.Methods A total of 87 patients of thoracolumbar fractures were treated with transpedicular intracorporeal grafting combined with pedicle screw fixation from January 2008 to De cember 2012.Preoperative,postoperative and follow-up VAS grade,kyphosis angle,complication and neurofunctional recovery were observed.Results All cases were followed up about 12 to 48 months,averagely 25.3 ±12.5 months.There were no failure cases and serious complications.In patients with incomplete injury,neurofunctions showed 1 to 2 grade improvement compared with the findings before operation.The postoperative and follow-up VAS grade were(5.3±0.7)cores and(2.8±0.5)cores,those were remarkable lower than preoperative VAS grade(P < 0.05).The postoperative and follow-up kyphosis angle were(-3.1±6.2)° and(0.7±0.4)°,those were remarkable lower than preoperative kyphosis angle(P < 0.05).Conclusion The transpedicular intracorporeal grafting combined with pedicle screw fixation was safe,reliable and fewer complications.It is a effective treatment method.更多还原
