血清N末端脑钠肽前体对维持性血液透析患者干体质量评估的价值
Application of receiver operating characteristic curve in the evaluation of serum NT-proBNP in predicting dry weight of maintenance hemodialysis patients
【摘要】 目的评价血清N末端脑钠肽前体(NT-proBNP)对稳定的维持性血液透析患者干体质量是否达标的预测价值。方法选择江门市中心医院血液净化中心稳定的血液透析患者73例,均进行规范透析治疗,依干体质量是否达标分为未达标组和达标组,未达标组35例,达标组38例。采用非参数检验和绘制ROC曲线评价血清NT-proBNP水平对干体质量评估的价值。结果于体质量未达标组患者血清NT-proBNP水平中位数为4256.78(3447.92~5612.33)pg/ml,干体质量达标组患者血清NT-proBNP水平中位数为2547.38(1968.84~3253.4)pg/ml,2组比较差异有统计学意义(P<0.01);NT-proBNP预测干体质量未达标的ROC曲线下面积为0.875±0.039(95%CI 0.798~0.952),以血清NT-proBNP水平>3257.56 pg/ml作为界值判断干体质量未达标的敏感性为80.0%,特异性为78.9%,2组NT-proBNP>3257.06 pg/ml者比例比较差异有统计学意义(P<0.01)。结论 NT-proBNP可作为维持性血液透析患者干体质量评估的参考指标之一。
【Abstract】 Objective To evaluate the value of NT-proBNP in predicting dry weight of stable maintenance hemodialysis patients by using the receiver operating characteristic(ROC) curve.Methods The data of 73 stable maintenance hemodialysis patients from September 2012 to December 2012 were retrospective reviewed.Dry weight was assessed and serum NTproBNP level was assayed by chemiluminescence.According to the results,the MHD patients were divided into uncontrolled group(n =35) and controlled group(n =38).The data were compared between two groups with nonparametric test and the ROC curve was drawn.Results There was significant difference in plasma NT-proBNP levels between uncontrolled group and controlled group 4256.78(3447.92~5612.33) pg/ml vs.2547.38(1968.84~3253.4) pg/ml.The area under curve of NT-proBNP to predict the uncontrolled dry weight was 0.875±0.039(95%CI 0.798~0.952) when the cut off level was 3257.56 pg/ml with the sensitivity of 80%and the specificity of 78.9%.The proportion of cases with NT-proBNP >3257.56 pg/ml was significantly different between the 2 groups(P <0.01).Conclusion The level of NT-proBNP has predictive value and may be used as one of the screening indicators for dry weight assessment.
