弥散张量成像对弥漫性轴索损伤的诊断价值
Application of Magnetic Resonance Diffusion Tensor Imaging in Diagnosis of Diffuse Axonal Injury
【摘要】 目的:探索磁共振弥散张量成像(DTI)对弥漫性轴索损伤(DAI)的评价价值。材料与方法:脑外伤病人45例及正常对照组12例,分别行常规MR扫描及弥散张量成像扫描。在FA图上分别测量创伤组及对照组双侧半球白质感兴趣区FA值,将两组相应感兴趣区FA值均值进行比较,观察有无明显差异。并将创伤组FA值均值与临床GCS评分和昏迷时间进行相关分析。结果:与对照组对比,创伤组胼胝体膝部、体部、压部及内囊后肢FA值均明显降低(P<0.01)。内囊后肢、胼胝体膝部、压部FA值的变化与GCS评分正相关,与昏迷时间负相关。结论:磁共振弥散张量成像技术对白质损伤敏感,并能反映其损伤程度,是目前活体诊断弥漫性轴索损伤准确的检查方法。
【Abstract】 Purpose: To explore the the value of magnetic resonance diffusion tensor imaging(DTI) in diffuse axonal injury(DAI).Materials and Methods: Prospectively routine MR and DTI were applied to 45 patients with traumatic head injury history and 12 normal adults to be the control group.Measure the FA value in bilateral ROI of trauma and control group,compare the mean FA value between the two groups,and correlate the mean FA value with the clinical glasgow coma score(GCS) and the time of coma in the trauma group.Results: Compared with control group,the mean FA values of corpus callosum and posterior internal capsule in DAI group significantly decrease(P < 0.01).The mean FA values of posterior internal capsule,genu and pad of corpus callosum were significantly correlated with GCS and the time of coma.Conclusion: DTI is sensitive to detect the traumatic injury of white matter and evaluate the degree of injury,it is a sensitive and accurate method in the diagnosis of DAI in vivo.
