依达拉奉对心室颤动大鼠心肺复苏后的脑保护作用

【摘要】 目的:研究依达拉奉对心室颤动大鼠脑组织的保护作用。方法:健康SD大鼠66只,随机分为3组:假手术组(n=18),盐水组(n=24),依达拉奉组(n=24),每组再分为自主循环恢复后12,24,48h3个时间点。盐水组与依达拉奉组均采用经食道快速起搏的方法制作大鼠心室颤动模型,常规行心肺复苏术,自主循环恢复即刻分别给予生理盐水或依达拉奉,其中依达拉奉为3mg/kg,生理盐水组输入等体积的生理盐水。监测血流动力学1h,结扎股动、静脉消毒缝合,放入笼内常规饲养,分别于上述3个时间点腹腔麻醉后快速断头取脑组织,左脑用于测定超氧化物(SOD)、丙二醛(MDA)含量;右脑行HE染色,光镜下观察海马CA2-3区病理形态学改变。结果:依达拉奉组大鼠脑组织中的SOD含量在12,24,48h均高于盐水组(P<0.05),MDA含量在以上3个时间点均低于盐水组(P<0.05或P<0.01);镜下观依达拉奉组海马组织病理损伤程度较盐水组为轻,以24,48h最为明显。结论:自主循环恢复后尽早使用依达拉奉,可通过提升脑内的SOD活力,降低MDA水平,减轻缺血再灌注损伤而起到脑保护作用。

【Abstract】 Objective:To investigate the protective effect of edaravone on brain in rats with ventricular fibrillation during cardiopulmonary resuscitation.Methods: Sixty-six healthy SD rats were randomized into sham(n=18),saline(n=24),and edaravone groups(n=24).Each group was then subdivided into 12 h,24 h and 48 h time points after restoration of spontaneous revascularization.Ventricular fibrillation models were produced through rapid transesophageal pacing method in both saline edaravone groups which were then followed by conventional cardiopulmonary resuscitation.As soon as the spontaneous revascularization was noticed,normal saline and edaravone were injected immediately in the corresponding group.Edaravone group was injected with 3 mg/kg of edaravone and saline group was infused with normal saline for the same volume as edaravone(3 mg/kg) had in total.Hemodynamic monitoring was performed for an hour,the femoral artery and vein after disinfection were sutured,and the rats were put back into the cage for conventional feeding.Rapid rats decapitation followed by brain tissue extraction was done after intraperitoneal anesthesia according to the above three time points.Left side of the brain was used to measure superoxide dismutase(SOD) and malondialdehyde(MDA) content and the right brain for HE stain and hippocampal CA2-3 pathomorphology observation under light microscope.Results: The rats in edaravone group had higher SOD and lower MDA contents than those in normal saline group at 24 h,48 h and 72 h time points(P<0.05 or P<0.01).The hippocampus in edaravone group had mild pathological damage as compared with saline group through the light microscope.Conclusion: The early use of edaravone after recovery of the spontaneous circulation in rats can minimize the effect of brain reperfusion injury through enhancing the SOD activity and decreasing the MDA level,by which,raising its protective role in cerebral ischemia.
