【摘要】 目的探讨显微手术治疗硬脊膜动静脉瘘(SDAVF)前辅以CT血管成像(CTA)检查的临床治疗效果。方法 52例SDAVF患者术前行CTA检查和脊髓血管造影(DSA),确诊后行显微手术治疗。以DSA为金标准,计算CTA诊断的敏感度。采用Aminoff和Logue评分标准评价患者手术前后的脊髓功能。结果 CTA对52例SDAVF均做出正确诊断,对供血动脉及瘘口诊断的敏感度为100%。随访中有42例(87.5%)术后症状改善,无症状加重及复发者。结论 CTA可以准确定位诊断SDAVF的供血动脉及瘘口,是手术前必要的辅助检查。显微手术直接夹闭硬脊膜动静脉瘘口,可取得良好的疗效。更多还原

【Abstract】 Objective To investigate the efficacy of microsurgery combined with CT angiography(CTA) on spinal dural arteriovenous fistula(SDAVF).Methods Fifty-two cases with SDAVF were diagnosed by CTA and selective spinal angiography were performed pre-operatively.Microsurgery was carried out for all patients.Taking DSA as the golden standard,the sensitivity of CTA was calculated.The function of the spinal cord was evaluated before and after treatment by using the Aminoff and Logue scale.Results All 52 cases were correctly diagnosed with CTA.The sensitivity of CTA in revealing the feeding artery and fistula of SDAVF was 100%.The postoperative neurological status was improved in 42 patients(87.5%),no deterioration or recurrence was observed.Conclusion CTA can locate the site of SDAVF clearly and microsurgery is an effective approach for the treatment of SDAVF.更多还原
