神经肽Y在突出腰椎间盘组织中的表达及意义

【摘要】 目的:探讨突出的腰椎间盘髓核组织中神经肽Y(neuropeptide Y,NPY)的表达及表达的意义。方法:收集30例有症状的腰椎间盘突出症患者,其中L4/5或L5/S1需行腰椎间盘髓核摘除术,取出腰椎间盘髓核组织作为实验组,同时收集20例尸体L4/5椎间盘组织作为对照组。将取出标本固定后切片染色,观察正常和病变椎间盘组织学改变。采用免疫组化方法对标本内NPY染色阳性的神经纤维进行定性和定量分析。结果:突出椎间盘的髓核组织中,少数细胞位于丰富的基质中。细胞膜不清楚,可见胞质内圆形细胞核及胞质内空泡。髓核中透明软骨细胞核大而胞浆深染,多聚集成团。突出的髓核组织中NPY的表达高于正常髓核组织,有显著性差异(t=17.617,P<0.05)。结论:在突出腰椎间盘的髓核组织中有大量的NPY表达。这说明交感神经纤维已经侵入到中央的髓核组织,释放出NPY递质,可能是椎间盘突出患者早期下腰痛的机制之一。

【Abstract】 Objective: To study the expressions and significance of neuropeptide Y(NPY) in intervertebral disk displacement.Methods: 30 nucleus pulposus(NP) of L4/5 or L5/S1 were included in treatment group,which were extirpated from patients with intervertebral disk displacement.20 NP of L4/5 were removed from corpse for control group.All the specimens were observed based on the morphologic features.The quantitative and qualitative analysis about the positive staining NPY nerve fibers were maked by immunocytochemistry.Results: a small number of cells were found in a rich matrix of displacement of the nucleus pulposus.It can be found that the cell membrane was not clear,and round nuclei and vacuoles were in the cytoplasm.hyaline cartilage cells had large nucleus,cytoplasm deeply stained and grouped.Expression of NPY in the herniated NP tissue was higher than that in the normal tissue(t = 17.62,P < 0.05).Conclusion: NPY overexpression was present in the herniated NP tissue.This shows that sympathetic nerve fibers invaded the central nucleus pulposus and released NPY neurotransmitter.This may be one of the mechanisms for low back pain of intervertebral disk displacement.
